صفحه 2مقدمهمیزان اضطراب و تنش در مادران دارای نوزاد نارس بیش از سایر مادران است و این مساله بر ارتباط مادر و کودک و پیامدهای تکاملی نوزاد در آینده تاثیر منفی دارد. تنش و اضطراب علاوه بر تاثیرات جسمی و روانی بر مادر، مانع ایجاد ارتباط مناسب و زودهنگام بین مادر و نوزاد شده و مشارکت مادر در امر مراقبت از نوزاد را نیز به تعویق می اندازد که این موارد بر پیامدهای تکاملی نوزاد در آینده موثر است.روش کاراین پژوهش یک کارآزمایی بالینی است که بر روی 90 مادر دارای نوزاد نارس در دو گروه مداخله و کنترل انجام شد. برنامه ایجاد فرصتهای توانمندسازی والدین یک برنامه آموزشی رفتاری چند مرحله ای است که طی آن اطلاعات لازم درباره نوزاد نارس و بخش مراقبت ویژه نوزادان از طریق نوار صوتی و نیز به صورت کتابچه مصور همراه با کتاب کار، به مادر داده می شود. میزان تنش و اضطراب مادران دو گروه، طی سه نوبت به فاصله 2 تا 4 روز و میزان مشارکت یک نوبت پس از انجام مداخله سنجیده شد.یافته هامیزان تنش و اضطراب مادران در دو گروه قبل از اجرای مداخله تفاوت معنی داری نداشت ولی بعد از انجام هر مرحله از برنامه، میزان تنش و اضطراب مادران در گروه مداخله کمتر از میزان تنش و اضطراب مادران در گروه کنترل گردید (P<0.001)، همچنین میزان مشارکت مادران در گروه مداخله بیشتر از میزان مشارکت مادران در گروه کنترل بود (P<0.001).نتیجه گیریبرنامه ایجاد فرصت های توانمندسازی والدین با ارائه شیوه متفاوتی از آموزش در بیمارستان، میزان اضطراب و تنش مادران دارای نوزاد نارس را کاهش و میزان مشارکت آنان را افزایش می دهد.

صفحه 8مقدمهبرنامه ریزان آموزش پرستاری معتقدند که مهمترین قسمت برنامه آموزش پرستاری، آموزش بالینی است. هدف از این مطالعه تعیین اولویت های موانع دستیابی به اهداف آموزشی کارآموزی های بالینی پرستاری از دیدگاه اساتید، سرپرستاران و دانشجویان پرستاری در دانشگاه علوم پزشکی جهرم می باشد.روش کارمطالعه حاضر یک مطالعه توصیفی-مقطعی است که توسط یک پرسشنامه محقق ساز انجام شده است. نمونه گیری به روش سرشماری بر روی تمامی دانشجویان پرستاری که حداقل یک دوره کارآموزی را گذرانده بودند (101 نفر)، همه مربیان پرستاری دانشکده پرستاری (9 نفر) و همه مدیران پرستاری بیمارستانهای دانشگاه علوم پزشکی جهرم (31 نفر) که حداقل 6 ماه سابقه مدیریت داشتند انجام گرفت. تجزیه و تحلیل اطلاعات به کمک نرم افزار SPSS 12.0 و با استفاده از روش های آماری انجام گرفت.یافته هاطبق دیدگاه دانشجویان مهمترین مانع دستیابی به اهداف آموزشی حضور تعداد زیاد دانشجویان در بخش با فراوانی پاسخ (5/52 درصد) بود. بر اساس دیدگاه مربیان مهمترین مانع دستیابی به اهداف آموزشی عبارت بودند از: بی علاقگی دانشجویان، نداشتن دید مثبت به آموزش توسط مدیران پرستاری، عدم استفاده از طرح درس در آموزش بالینی دانشجویان همگی با فراوانی پاسخ یکسان (7/66 درصد) و در نهایت از دیدگاه مدیران پرستاری عدم علاقه مربیان پرستاری به کار پرستاری با فراوانی پاسخ (6/51 درصد) به عنوان مهمترین مانع دستیابی به اهداف آموزشی مطرح گردید.نتیجه گیریتوجه به عواملی مانند؛ تعداد زیاد دانشجویان، بی علاقگی مربیان و دانشجویان به حرفه پرستاری و عدم استفاده از طرح در آموزش بالینی و نداشتن دید مثبت به آموزش توسط مدیران پرستاری به عنوان مهمترین موانع دستیابی به اهداف آموزشی کارآموزی های بالینی می تواند در برنامه ریزی های ارتقاء کیفیت آموزش بالینی کمک کننده باشد.

صفحه 14مقدمهپرستاران 50 تا 60 درصد نیروی انسانی تخصصی در بیمارستانها را تشکیل می دهند و قسمت عمده مراقبت از بیماران به عهده آنها می باشد. بنابراین اهمیت رهبری آگاهانه در خدمات بهداشتی درمانی خصوصا در سطح پرستاری به خوبی آشکار است. مطالعه حاضر با هدف بررسی ارتباط بین سبک های رهبری سرپرستاران با رضایت شغلی پرستاران شاغل در بخش های داخلی جراحی انجام شد.روش کارمطالعه به صورت توصیفی – تحلیلی بر روی 64 نفر از پرستاران شاغل در بخش های داخلی جراحی بیمارستانهای وابسته به دانشگاه آزاد اسلامی واحد تنکابن انجام شد.نمونه گیری به روش سرشماری و از پرستارانی که حداقل 3 ماه سابقه کار در بخشهای مذکور را داشتند، صورت گرفت. ابزار گرداوری داده ها پرسشنامه ای 3 قسمتی مشتمل بر مشخصات فردی پرستاران و سرپرستاران، پرسش نامه توصیف رفتار رهبری سرپرستاران (پرسش نامه دانشگاه ایالت اوهایو1957) و پرسش نامه مربوط به سنجش رضایت شغلی پرستاران شاغل (مقیاس رضایت شغلی موهرمن-کوک-موهرمن،1987) بود.یافته هااکثریت واحد های دارای رضایت شغلی بالا (5/80 درصد) افرادی بودند که سرپرستاران آنها از سبک رهبری متقاعد کننده استفاده می کردند و بر عکس اکثریت افراد دارای رضایت شغلی پایین (7/66 درصد) پرستارانی بودند که سرپرستارانشان از سبک رهبری تفویضی برای سرپرستی بخش استفاده می کردند.نتیجه گیریدر سازمانهایی که مدیران علاوه بر بعد وظیفه مداری به بعد رابطه مداری هم توجه بالایی دارند و با ایجاد محیط مشاوره ای سعی می کنند کارکنان را در تصمیمات مشارکت دهند میزان رضایت شغلی بالا می رود که این خود منجر به افزایش تعهد سازمانی در کارکنان، افزایش کیفیت ارائه خدمات و در نهایت بالا رفتن رضایت در بیماران میشود.

صفحه 22مقدمهپرستاران کسانی هستند که در نتیجه مواجهه مکرر با تعداد زیادی از افراد بیمار و مرگ و میر آنان، با خطر درگیری عاطفی زیادی روبرو هستند. پریشانی اخلاقی هنگامی بوجود می آید که فرد می داند عمل درستی که باید انجام دهد چیست، اما مجموعه ای از عوامل و موانع او را متقاعد می سازند که انجام عمل درست، غیر ممکن است. پس می توان گفت پریشانی اخلاقی با بی میلی در کار،عدم رضایت شغلی، کاهش تعامل با بیماران و خانواده هایشان و در نهایت فرسودگی شغلی در ارتباط است. این مطالعه با هدف تعیین تاریخچه مفهوم پریشانی اخلاقی و عوامل تاثیر گذار بر آن در جامعه پرستاران با مرور دیگر مطالعات انجام شده است.روش کاربرای گرد آوری اطلاعات در زمینه پریشانی اخلاقی در پرستاری علاوه بر مطالعات کتابخانه ای از پایگاه های اطلاعاتی مدلاین، ovid proquest، پایگاه اطلاعات علمی جهاد دانشگاهی و ایران مدکس استفاده شد، مقالات از سال 1996 تا سال 2010 مورد بررسی قرار گرفتند از کلید واژه های Moral distress و پریشانی اخلاقی جهت جستجوی مقالات استفاده شد. در نهایت 23 مقاله مرتبط با پریشانی اخلاقی به مطالعه حاضر وارد شدند و مورد بررسی قرار گرفته اند.یافته هامطالعات مختلف نشان دادند که، عوامل موثر و ایجاد کننده پریشانی اخلاقی در پرستاران با توجه بر مرور متون شامل: «ارتباط مستقیم با بیمار»، «منابع ناکافی»، «فراهم کردن مراقبت های پزشکی بیهوده»، «بی حرمتی به استقلال بیماران»، «روابط بین پزشکان و پرستاران» و «ناتوانی در جلوگیری از مرگ» می باشد که همه ی آنها به نوعی می توانند بر عملکرد بالینی پرستاران و کیفیت مراقبت از بیمار تاثیر بگذارند.نتیجه گیریبا توجه به تاثیر و پیامدهای پریشانی اخلاقی بر پرستاران، مدیران پرستاری باید برای مقابله با پریشانی اخلاقی پرستاران، بروی منابع تمرکز کنند. همچنین به کار گماشتن پرستاران در کمیته اخلاق بیمارستان یکی از راه حل های موجود می تواند باشد.

صفحه 29مقدمهتغذیه با شیر مادر دارای فواید فراوان روانی، بهداشتی، ایمنی، اقتصادی برای نوزاد و مادر می باشد. با توجه به این که شیر مادر نقش عظیمی در ارتقاء سلامت و بهبود رشد و تکامل کودکان دارد. در این مقاله ضرورت اخلاقی تغذیه با شیر مادر در احیای حقوق نوزاد حاصل از روش-های نوین باروری مورد مطالعه قرار می گیرد.روش کاردر این مطالعه مروری به منظور دستیابی به تحقیقات انجام شده در زمینه مذکور سایت های ملی جهاد دانشگاهی، بین المللی گوگل پژوهشگر و کتابخانه بین المللی آمریکا با دادن کلیدواژه های مذکور در فاصله زمانی نامحدود جستجو و نتایج بدست آمده با یکدیگر ترکیب شدند.یافته هازایمان های چند قلویی و تولد نوزادان نارس و کم وزن بزرگترین مشکل در نوزادان حاصل از درمان های کمک باروری است. بنابراین درصد نارسی و وزن کم 4 تا 5 برابر بیشتر از جمعیت عمومی می باشد.. نتایج مطالعات حاکی از این است که شانس بروز عفونت حاد تنفسی شیرخواران در گروهی که تغذیه انحصاری با شیر مادر داشتند 62 درصد کمتر از شیر خوارانی بود که تغذیه انحصاری با شیرمادر نداشتند. روش های مختلفی جهت تهیه شیر از طریق بانک شیر مادر، گرفتن دایه و القا شیردهی و در نهایت استفاده از شیر خشک جهت تغدیه نوزاد وجود دارد.نتیجه گیریبا توجه به اینکه شیر دادن به کودک زیربنا و اساس شخصیت او را پی ریزی می نماید. افزایش ضریب هوشی کودک، تسهیل تربیت و کاهش بروز مشکلات جسمی، اختلالات رفتاری، بزهکاری نوجوانی و اعتیاد در بزرگسالی از فواید تغذیه با شیر مادر است با مرور مقالات مربوطه به نظر می رسد روش گرفتن دایه نسبت به سایر روش های مذکور ارجحیت بیشتری داشته باشد.

صفحه 40مقدمهامروزه به دلیل استفاده از تکنولوژی های پیشرفته در درمان های پزشکی و تحولات پیچیده اجتماعی، مساله اخلاق جایگاه ویژه ای در علوم پزشکی دارد در نتیجه آموزش آن الزامی است. اخیرا در ایران، آموزش اخلاق پزشکی بر اساس آموزش های دینی بسیار مورد توجه قرار گرفته است. از این جهت، اخلاق پزشکی به عنوان نظام کاربردی همواره به دنبال راه کارهای جدید برای رفع مشکلات اخلاقی بوده است.
روش کارروش تحقیق در این مقاله، به صورت کیفی از نوع کاربردی- تبیینی بوده و همچنین اهمیت و مراحل پیشرفت آموزش اخلاق پزشکی در ایران نیز مورد بررسی قرار گرفت.یافته هااولین هدف آموزش اخلاق پزشکی، تقویت توانایی های کارآموزان در انجام مراقبت های بهداشتی از بیماران و حمایت از خانواده های ایشان می باشد با این وجود مسئله اصلی تنها شخصیت سازی نیست بلکه در حل یک مسئله اخلاقی، شناخت موضوع اخلاق و به کارگیری دانش مناسب با موضوع، آنالیز مسئله و تصمیم گیری درباره روند اقدامات نیز باید مورد توجه قرار گیرد. از این جهت، مشارکت فعالانه کارآموزان در روند آموزش حائز اهمیت می باشد.
نتیجه گیریآموزش اخلاق پزشکی بر پایه تدریس نظری (روش سنتی) به تنهایی نمی تواند نیازهای سیستم بهداشتی-درمانی کشور را جهت داشتن پزشکان متعهد و متخصص برآورده سازد در نتیجه ایجاد تحول و استفاده از روش های جدید آموزش که مبتنی بر جامعه است می تواند هم به سود حرفه پزشکی و هم به سود بیماران باشد. از این جهت، در این مقاله پس از بررسی اهمیت و وضعیت فعلی آموزش اخلاق پزشکی در کشور، پیشنهاداتی در جهت اصلاح ساختار آموزشی ارائه شده است.

Page 2IntroductionMothers who have premature infants experience higher levels of anxiety and tension. This affects the relation of mother to the infant and bears on negative developmental consequences for future. Besides its physical and psychological impacts, anxiety and tension impedes the prompt and suitable relationship between the mother and baby and also defers mother’s attendance to the care of infant. This will have negative consequences for infant’s developmental processes.Materials And MethodsThis is a clinical trial implemented on 90 mothers with a premature infant divided into intervention and control groups. The program for creating parental rehabilitative opportunities is a step by step program through which mothers are given necessary information regarding premature infant and the infants care unit by tapes, illustrated books and also workbooks. The level of mothers’ tension and anxiety was measures three times with intervals of 2-4 days.ResultsThe amount of anxiety and tension in the two groups did not show a significant difference, but the mothers’ anxiety and tension in intervention group reduced in each step (P<0.001). The level of mothers’ participation was also higher in the intervention group (P<0.001).ConclusionThe program for creating parental rehabilitative opportunities reduces the amount of mothers’ tension and anxiety and increases their level of participationKeywords: Psychological Criteria, Random Clinical Trial, Participation, Parents, Premature Infant

Page 8IntroductionNursing education planners believe the most important part of nursing education is clinical education. This study aimed to determine the importance of obstacles in obtaining clinical training goals from the prospective of nursing educators, nursing students and nursing managers of Jahrom University of Medical Sciences.Materials And MethodsThis cross-sectional study was performed by a self-report questionnaire. Sampling was performed by census and the samples consisted of all nursing students who had passed at least one course of clinical education (n: 101), all nursing educators (n: 9) and all nursing managers who had management experience for at least for 6 months (n=31). Data analysis was performed by SPSS 12.0 and statistical methods.ResultsThe most important obstacle to obtain educational goals was the presence of many students in wards (52.5%) according to the students’ views. In educators’ view the most important obstacles were students’ disinterest in nursing, nursing managers’ lack of positive attitude toward education, and the lack of course plan in clinical education (66.7%). Finally, educators’ disinterest in nursing profession was rated as the most important obstacle by the nursing managers.ConclusionAttending to factors such as the large body of students, educators’ and students’ disinterest toward nursing profession, and the lack of course plan in clinical education, and nursing managers’ lack of positive attitude toward education which are regarded as the most serious obstacles in reaching the educational goals of practitioners’ clinical education can contribute to the quality of future clinical education.Keywords: Nursing, Clinical Training, Obstacles

Page 14IntroductionNurses comprise 50% to 60% of specialists working in hospitals and are responsible for the main parts of the patient care. The importance of leadership in health organizations especially in the field of nursing is therefore obvious. This study aimed to evaluate any possible relation of leadership style among head nurses to job satisfaction among nurses working in medical and surgical wards.Materials And MethodsThis descriptive-analytical study was performed on 64 nurses working in medical and surgical wards of hospitals affiliated to Tonekabon Islamic Azad University. Sampling was performed by census. Data were gathered using a questionnaire consisting of three parts: 1) personal details, 2) description of leadership behavior of head nurses (University of Ohio, 1957), 3) measurement of nurses’ job satisfaction (job satisfaction Index by Mohrman-cooke-Mohrman, 1987).ResultsMost samples who showed a high job satisfaction (80.5%) were those whose head nurses manipulated an acceptable convincing leadership style.; on the other hand, those who showed a low job satisfaction (66.7%) were mostly nurses whose head nurses consigned their responsibilities.ConclusionThe organization in which the managers pay attention to establishing good relations with the staff as well as the duties they have to do, and try to make the staff contributing to decision making, job satisfaction increases to a high level which will result in the elevation of the level of organizational commitment, quality of services and satisfaction among patients.Keywords: Leadership, Job Satisfaction, Nursing

Page 22IntroductionNurses are disposed to emotional conflicts as they frequently face large numbers of patients and witness their subsequent deaths. Moral distress appears when the person knows what they ought to do but a set of factors and obstructions convince them that doing the proper action is not possible. Moral distress is thus related to professional reluctance, lack of job satisfaction, reduction of interaction with patients and their families, and ultimately professional exhaustion. This study aimed to determine the history of the concept of ‘moral distress’ and the factors affecting it in nurses’ society by reviewing other studies.Materials And MethodsResearchers used Medline, Proquest, Ovid, SID and IranMedex databases in addition to library research. Articles published in 1996 to 2010 were studied. ‘Moral distress’ was used as a keyword to find relevant articles. 23 articles related to moral distress were eventually included in the present study.ResultsVarious studies revealed that the factors affecting moral distress include: ‘direct contact with patient’, ‘inadequate resources’ ‘provision of vain medical care’, ‘lack of respect toward the patients’ independence’, ‘relationships among physicians and nurses’ and ‘inability to prevent death’ which all can affect the nurses’ clinical performance and the quality of healthcare.ConclusionRegarding the effects of moral distress on nurses, nurse managers should focus on resources to confront moral distress. Page 29IntroductionBreastfeeding has many psychological, immunological and economic benefits for the mother and her infant. As mother’s milk plays a great role in the enhancement of health and the elevation of children’s growth, this paper studies the necessity of breastfeeding in reviving the rights of infants born by the new fertilization techniques.Materials And MethodsSID, Google Scholar and International Library of America were used to access research findings in relevant fields for this review study. The mentioned keywords were searched in an unlimited time span and results were compared to each other.ResultsMultiple deliveries, premature and underweight births were the major problems in infants born by new fertilization techniques. The percentage of prematurity and underweight were 4-5 times higher than general population. Findings reveal that acute respiratory infection in infants who were exclusively breastfed was 62% less compared to those who were not exclusively breastfed. There are various ways for getting milk like getting milk from mother milk bank, employing nurses, induced lactation, and eventually using powder milk.ConclusionLactation underlies the cornerstones of one’s personality; breastfeeding can thus lead to the elevation of IQ, facilitation of education, decrease in incidence of physical problems and behavioral disorders, encumbering adolescent’s misdemeanors and adult addiction. Page 40IntroductionToday, advanced technology in medical treatment involves in the evolution of complex social and which gives medical ethics a special status and makes it indispensable for medical training. Medical ethics based on religious teachings has been recently brought to the fore. In this regard medical ethics, as an applied system, has always sought new solutions for ethical problems.Materials And MethodsThe method for this research was qualitative, using an applied interpretive approach while focusing on the significance and stages of development in education of medical ethics.ResultsThe first aim of medical ethics is to elevate the practitioners’ ability to give health care to patients and support their families. However it is not just a development of personality, but the recognition of ethics, using the appropriate relevant knowledge, analysis of the problem and making decisions about the process should also be considered. Thus, students’ active participation in the learning process is very important.ConclusionsMedical ethics education based on theoretical training (traditional approach) cannot meet the health system’s needs for responsible and professional physicians. Therefore changing the current methods of training and using new methods based on society can benefit both the health profession and patients. After a reviewing the current status of medical ethics education in our country, this paper suggests solutions for reforming the educational structure.Keywords: Medical Ethics, Education, Medical Sciences
