صفحات 195-205مقدمهیکی از روش های کمکی برای کنترل پوسیدگی، مواد ضد باکتریایی می باشد. بدنبال بروز عوارض جانبی و مقاومت به مواد ضد باکتریایی شیمیایی، استفاده از گیاهان دارویی مورد توجه است. در این مطالعه اثر ضدباکتریایی افشره زیره سبز و اکالیپتوس بر سه سوش لاکتوباسیل پوسیدگی بررسی شد.مواد و روش هادر این مطالعه تجربی آزمایشگاهی استخراج افشره با روش تقطیر با آب انجام و با دستگاه گازکروماتوگرافی- طیف نگار جرمی، ترکیب شیمیایی آنها ارزیابی شد. شاخص های فعالیت ضد باکتریایی شامل MIC، MBC و قطر هاله عدم رشد افشره هابر سه سوش لاکتوباسیل (کازئی، پلانتاروم و رامنوسوس) به دو روش ماکرودیلوشن براث و انتشار چاهکی در آگار اندازه گیری شد. نتایج با استفاده از آزمون های واریانس یکطرفه و توکی تجزیه و تحلیل گردیدند (05/0α=).یافته هااصلی ترین ترکیبات افشره زیره سبز و اکالیپتوس به ترتیب کومینیک الکل (%23/30) و 8،1-سینئول(%18/41) بودند. MIC هر سه سوش باکتری برای زیره μg/ml71/6 و برای اکالیپتوس μg/ml110 بود. MBC برای لاکتوباسیلوس کازئی، لاکتوباسیلوس پلانتاروم و لاکتوباسیلوس رامنوسوس به ترتیب μg/ml43/13، μg/ml87/26 و μg/ml87/26 برای زیره و μg/ml110، μg/ml220 و μg/ml110 برای اکالیپتوس بود. افزایش غلظت افشره ها موجب افزایش قطر هاله عدم رشد شد (0 (p value=، وبیشترین قطر هاله عدم رشد در هر دو افشره مربوط به غلظت 880 میکروگرم بر میلی لیتر بود. از میان سه گونه باکتری مورد مطالعه، L. Casei بیشترین حساسیت و L. Plantarum کمترین حساسیت را نسبت به افشره ها نشان داد (02/0 (p value=. قطر هاله عدم رشد در زمان های مختلف (24و 48 ساعت) نشان می دهد که قطر هاله عدم رشد طی زمان تغییر معنی داری نداشته است (78/0 (p value=. قطر هاله عدم رشد برای افشره زیره سبز بیشتر از اکالیپتوس است (02/0(p value=نتیجه گیرینتایج مطالعه نشان داد که هر دو افشره بر سوش های مورد مطالعه اثر ضد باکتریایی دارند ولی زیره سبز تاثیر بیشتری دارد.

صفحات 206-215مقدمهدسترسی و توزیع یکسان خدمات بین همه ی افراد، به عنوان یکی از اهداف اصلی سرویس های سلامت هر جامعه ای است و رضایتمندی بیمار فاکتور مهمی در ارزیابی این اهداف می باشد. این مطالعه با هدف بررسی دسترسی به خدمات دندانی و نیز رضایتمندی از این خدمات از دیدگاه والدین کودکان 14-6 ساله شهر اصفهان در سال 1390 صورت گرفت.مواد و روش هااین مطالعه ی توصیفی بر روی 390 نفر از والدین کودکان 14-6 ساله ی ساکن شهر اصفهان و از طریق پرسشنامه و مصاحبه صورت گرفت. محتوای پرسشنامه شامل خصوصیات دموگرافیک، بررسی کلی وضعیت سلامت دهانی کودکان، اطلاعات کلی مرتبط با وضعیت رفتار های سلامت، شاخص های دسترسی به خدمات، موانع مربوط به دسترسی و ارزیابی میزان رضایتمندی از این گونه خدمات بود. داده ها توسط روش های آماری توصیفی وآنالیز رگرسیون خطی تحلیل گردید (05/0 =α).یافته هاطبق گزارش والدین در این مطالعه 78%از کودکان دسترسی به خدمات سلامت دهان و دندان داشتند، 1/22% دچارموانع بوده اند و شایع ترین موانع هزینه ی بالای درمان، عدم پوشش بیمه و ترس فرزندان از دندانپزشکی بود.48%از افراد از دسترسی به خدمات سلامت دهان راضی بودند. علت آخرین مراجعه به دندانپزشکی در 68% کودکان مشکلات مربوط به لثه و دندان و 22% به علت معاینات منظم دندانی ارزیابی گردید.نتیجه گیریطبق این مطالعه دسترسی به خدمات دندانپزشکی از دیدگاه والدین بالای 70% بوده و مشکل اصلی والدین هزینه های بالای درمان، عدم پوشش بیمه ها وترس از دندانپزشکی فرزندان می باشد. نیمی از والدین از خدمات دریافت شده توسط فرزندشان رضایت دارند.

پوسیدگی دندانی از شایع ترین بیماری های مزمن دوران کودکی است. بعضی از مطالعات تاثیر مخرب سیگار کشیدن فعال و همچنین دود سیگار محیطی را بر پیشرفت پوسیدگی دندانی گزارش نموده اند. هدف از این مطالعه مقایسه میزان پوسیدگی دندانی با استفاده از شاخص های dft و DMFT Decay-missing-filling-teeth)) بین کودکان در معرض دود سیگار محیطی و کودکانی که در معرض دود سیگار نیستند، بود.
مواد و روش هادر این تحقیق توصیفی تحلیلی، 90کودک 12-8 ساله مورد مطالعه قرار گرفت. کودکان بر اساس این که تحت تاثیر دود سیگار قرار گرفته اند یا نه، به دو گروه مساوی تقسیم شدند. در هر دو گروه پوسیدگی دندانی، دندان های از دست رفته و دندان های ترمیم شده با استفاده از معاینه تعیین شد و در پرسشنامه ثبت گردید و شاخص های dft و DMFT برای هر دو گروه محاسبه شد. همچنین ایندکس پلاک کل دهان کودکان تعیین شد و میزان آنزیم کوتینین بزاق با استفاده از روش الایزا اندازه گیری و ثبت گردید و داده ها با آزمون t مستقل تجزیه وتحلیل گردید. (05/0= α).
یافته هامیزان شاخص dft در کودکانی که در معرض دود سیگار محیطی هستند و کودکانی که تحت تاثیر دود سیگار نمی باشند، با هم تفاوت معنی دار نداشت (35/0Pvalue=) و میزان شاخصDMFT در دو گروه مورد و شاهد، با هم تفاوت معنی دار نداشت (61/0Pvalue=) و همچنین میزان کوتینین بزاق در دو گروه مورد مطالعه با هم تفاوت معنی دار نداشت (96/0=Pvalue)
نتیجه گیریکودکانی که در معرض دود سیگار محیطی قرار داشتند نسبت به کودکانی که تحت تاثیر دود سیگار محیطی نبودند به یک اندازه امکان ابتلا به پوسیدگی دندان را داشتند.

صفحات 223-229مقدمهشیوع یافته های تصادفی در سینوس ماگزیلا بین 6/23% تا 44% گزارش شده است. هدف از این مطالعه تعیین شیوع یافته های تصادفی در سینوس ماگزیلا در آرشیو(CBCT) Cone Beam Computed Tomography دانشکده دندانپزشکی در اصفهان بود.مواد و روش هادر این مطالعه مقطعی-توصیفی گذشته نگر تعداد 100 تصویرCBCT که در آن 4 دیواره سینوس های ماگزیلاری بیماران بالای 12 سال قابل بررسی بود، از آرشیو بخش رادیولوژی دانشکده دندانپزشکی اصفهان مورد مطالعه قرار گرفتند. تصاویر توسط یک نفر دستیار رادیولوژی دهان و فک و صورت توسط یک رایانه با شدت ثابت نور و رزولوشن و در ساعات 10-8 صبح از نظر افزایش ضخامت مخاط، حضور کیست و سپتا بررسی شدند. ضایعات بر اساس مکان به یک طرفه و دو طرفه تقسیم شدند. در نهایت داده ها با استفاده از آزمون های آماری توصیفی و آزمون chi square و t-test مورد تجزیه و تحلیل قرار گرفتند (05/0=α).یافته هااز میان 100 بیمار مورد مطالعه در 76% موارد یافته های اتفاقی در حداقل یکی از سینوس ها کشف گردید و تنها 24% موارد فاقد یافته های اتفاقی بودند. شایع ترین یافته اتفاقی در سینوس ماگزیلا ضخیم شدگی مخاط(%63) و پس از آن به ترتیب کیست(%41) و سپتوم (%18) بود. نتایج آزمون chi square نشان داد که بین توزیع فراوانی ضخیم شدگی مخاط، کیست و سپتوم در سینوس میان زنان و مردان اختلاف معنی داری وجود نداشت، (به ترتیب)(584/0=pvalue، 172/0=pvalue و 71/0=pvalue).
آنالیز واریانس یک طرفه نشان داد که بین میانگین سن و ضخیم شدگی مخاط، کیست و سپتوم در سینوس تفاوت معنی دار وجود نداشت (949/0=pvalue، 755/0=pvalue و 606/0=pvalue).نتیجه گیریضخیم شدگی مخاط شایع ترین یافته اتفاقی سینوس ماگزیلا بود و پس از آن به ترتیب کیست و سپتا قرار داشتند. سن و جنس تاثیری بر رخداد ضایعات سینوسی نداشتند.

صفحات 230-238مقدمهسیل آپیکالی نقش مهمی در موفقیت درمان کانال ریشه دارد و لایه اسمیر باعث کاهش سیل آپیکالی می شود. هدف این مطالعه بررسی آزمایشگاهی تاثیر تابش لیزر (Neodymium- doped yttrium aluminum garnet) Nd:YAG بر سیل آپیکالی، در دندان های درمان ریشه شده، با استفاده از روش filtration fluid بود.مواد و روش هادر این مطالعه آزمایشگاهی تعداد 44 دندان سانترال ماگزیلای انسانی انتخاب شد. بعد از آماده سازی، نمونه ها به صورت تصادفی به چهار گروه: گروه A: شاهد (n=20)، گروه B: لیزر ((n=20، گروه C: کنترل مثبت (n=2) و گروه D: کنترل منفی (n=2) تقسیم شدند.
تمامی گروه ها به روش Crown down آماده سازی شدند. گروه B قبل از پرکردن تحت تابش لیزر Nd:YAG قرار گرفت. گروه های A،B و Dبوسیله گوتا پرکا و سیلر و گروه C با گوتا پرکا و بدون سیلر پر شدند. انتهای کرونالی نمونه ها توسط آمالگام سیل و پس از آن به مدت یک هفته در انکوباتور نگه داری شدند. سپس سطح خارجی نمونه های گروه های A، B و Cرا به جز دو میلی متر آپیکالی آن ها و نمونه های گروه D را تماما توسط دو لایه لاک و یک لایه موم چسب مذاب پوشاندیم. در ادامه میزان ریز نشت آپیکالی با روش fluid filtration سنجیده شد. داده ها به وسیله آزمون آماری کولموگروف- اسمیرنوف و آزمون t مستقل آنالیز شدند (05/0=α).یافته هامیانگین ریز نشت آپیکالی در گروه لیزر و شاهد به ترتیب 88/2±35/4 و 13/3±34/5 μL/min/cm H2O به دست آمد، که از لحاظ آماری معنی دار نبود (305/0P-value=).نتیجه گیریبا توجه به محدودیت های پژوهش حاضر نتایج مطالعه فعلی نشان داد که کاربرد لیزر Nd:YAG در درمان کانال ریشه بر روی میزان ریز نشت آپیکالی تاثیری ندارد.

صفحات 239-247مقدمهفعالیت های ورزشی یکی از عوامل اتیولوژیک اصلی آسیبهای ناشی از ضربه به دندان ها محسوب میشوند. هدف از این مطالعه بررسی میزان آگاهی ورزشکاران و مربیان ورزشی در برخورد با صدمات ناشی از ضربه به دندان ها بود.مواد و روش هااین مطالعه مقطعی بر روی ورزشکاران حرفه ای ورزش های تماسی و مربیان آنها در شهر مشهد و به صورت سرشماری انجام شد. از پرسشنامه ای شامل سوالاتی در مورد پروفایل شخصی و کاری پاسخ دهندگان و سوالاتی درباره چگونگی برخورد با چهار مورد ترومای دندانی استفاده گردید. اطلاعات با استفاده از تست های آماری Chi-squareو Mann-Whitney در سطح معنی داری 05/0=α آنالیزگردید.یافته هااغلب ورزشکاران (2/44%) سن بین 30-20 سال و اغلب مربیان (44%) سن بالای40 سال داشتند. سابقه فعالیت ورزشی در اکثر ورزشکاران بین 5-1 سال بود (37%) و در مربیان بیشتر از 10 سال بود (6/36%). میزان آگاهی ورزشکاران در مورد نحوه برخورد با صدمات دندانی در8/3 درصد افراد متوسط، 45 درصد کم، 8/43 درصد خیلی کم بوده است و 5/7 درصد هیچگونه آگاهی در این مورد نداشته اند. در مربیان میزان آگاهی در6/6 درصد افراد زیاد، 1/17 درصد متوسط، 9/53 درصد کم، 1/17 درصد خیلی کم بوده است و 3/5 درصد هیچگونه آگاهی نداشته اند. همچنین ارتباط معنی داری بین میزان آگاهی با جنسیت، گروه سنی و سابقه فعالیت در ورز شکاران و مربیان ورزشی وجود نداشت. Pages 195-205IntroductionAntibacterial agents are considered one of the adjuncts for caries prevention. Due to side effects and bacterial resistance with the use of chemical agents, use of herbal medications has attracted a lot of attention. This study evaluated the antibacterial effects of essential oil extracts of C. cyminum L. and E. globulus against three common oral lactobacilli involved in tooth caries.Materials And MethodsIn this study, essential oils were extracted by hydrodistillation. C. cyminum L. and E. globulus were characterized by means of gas chromatography‒mass specrtophotometry (GC‒MS). Antibacterial activity indices including MIC, MBC and zone of inhibition for the above essential oils against three bacterial strains (Lactobacillus casei, Lactobacillus plantarum and Lactobacillus rhamnosus) were determined using broth macrodilution and agar well diffusion methods. Data analysis was performed using one-way ANOVA and Tukey HSD tests (α=0.05).ResultsThe main components of the essential oils of C. cyminum L. and E. globulus were cuminic alcohol (30.23%) and 1.8 cineol (40.18%), respectively. MIC for all the bacterial strains was 6.71 μg/mL for C. cyminumand and 110 μg/mL for E. globulus. MBC values for L. casei, L. plantarum and L. rhamnosus were 13.43 μg/mL, 26.87 μg/mL and 26.87 μg/mL for C. cyminum and 110 μg/mL, 220 μg/mL and 110 μg/mL for E. globulus, respectively. An increase in the concentration of essential oils resulted in an increase in zone of inhibition (p value = 0). In both essential oils the maximum zone of inhibition was observed at a concentration of 880 mg/mL. L. casei had maximum and L. plantrum had minimum sensitivity to these oils (p value = 0.02). Zones of inhibition at different time intervals (24-48 h) showed no significant differences (p value = 0.78). Zone of inhibition of C. cyminum was more than that of eucslyptus (p value = 0.02).ConclusionBoth essential oils exhibited antibacterial effects against the bacterial strains evaluated. Pages 206-215IntroductionAccess to and equal distribution of healthcare services are one of the main goals of any public health service and the patient satisfaction is an important factor in evaluating these goals. The purpose of this study was to assess barriers to the accessibility of and the satisfaction of parents of 6‒14-year-old children with these services in Isfahan in 2011.Materials And MethodsThis descriptive study was carried out on 390 Isfahani parents with 6‒14-year-old children. Data was collected through a questionnaire and interviews. The questionnaire consisted of questions on demographic data, the general oral health status in children, the general healthcare behavior, barriers to access to dental services, availability of and access to such services and parental satisfaction with dental services in their children. Data were analyzed with descriptive statistics and linear regression analysis using SPSS software (α=0.05).ResultsBased on this study, 78% of children had access to dental care and 22.1% faced barriers, with the main barriers consisting of the high cost of dental treatments, insufficient dental insurance coverage and dental fears in children. The average satisfaction rate with dental services was 48%. Dental visits in 22% were due to regular dental checkups and 68% for dental and gingival problems.ConclusionAccording to this study, the rate of access to dental care in the samples was over 70%. The main barriers to access to dental services based on parents’ reports were the cost of dental care, insufficient dental insurance coverage and dental fear. Dental caries is the most common chronic disease in childhood. Some studies have reported the harmful effects of active smoking and environmental tobacco smoke on the development of dental caries. The aim of this study was to compare the caries rate using DMFT and dmft indexes in children exposed to environmental tobacco smoke with that in children not exposed to environmental tobacco smoke.
Materials And MethodsIn this descriptive study 90 children aged 8‒12 were studied. The children were divided into two equal groups based on being exposed or not being exposed to tobacco smoke. In both groups, dental caries, missed teeth and filled teeth were determined using examination and recorded in questionnaires, followed by calculation of DMFT and dmft indexes for both groups. In addition, the full-mouth plaque index was determined, along with salivary cotinine enzyme levels using ELISA technique. Data were analyzed with independent t-test (α=0.05).
ResultsThere were no significant differences in dmft index between children exposed to environmental tobacco smoke and children not exposed to tobacco smoke (p value = 0.35); in addition, there were no significant differences in DMFT index between the case and control groups (p value = 0.61). There were no significant differences in cotinine salivary levels between the two groups under study (p value = 0.96).
ConclusionThe children exposed to environmental tobacco smoke exhibited susceptibility to dental caries comparable to that in children not exposed to tobacco smoke.

Pages 223-229IntroductionThe prevalence of accidental findings in maxillary sinus has been reported to be 23.6‒44%. The aim of this study was to evaluate the prevalence of accidental findings in maxillary sinus in the CBCT archives of Isfahan Faculty of Dentistry.Materials And MethodsIn this retrospective, descriptive, cross-sectional study 100 CBCT images in which the 4 walls of the maxillary sinus were visible, in patients over 12 years of age, were selected. All the samples were evaluated by an oral and maxillofacial radiology resident in relation to mucosal thickening, cysts and septa using a computer with uniform resolution and light intensity from 8 to 10 in the morning. The lesions were divided into unilateral and bilateral depending on their location. Data were analyzed with SPSS 20 using descriptive methods, chi-squared test and t-test (α=0.05).ResultsOf 100 patients evaluated, 76% had accidental findings in at least one sinus and only 24% had no accidental finding. Mucosal thickening was the most common accidental finding (63%), followed by cysts (41%) and septa (18%). Chi-squared test showed no significant differences between males and females in the frequencies of mucosal thickening, cysts and septa (p values = 0.584, 0.172 and 0.71, respectively). One-way ANOVA showed no significant relations between age and the prevalence of mucosal thickening, cysts and septa (p values = 0.949, 0.755 and 0.606, respectively).ConclusionsMucosal thickening was the most common accidental finding in the maxillary sinus, followed by cysts and septa, respectively. Pages 230-238IntroductionGood apical seal plays a critical role in the success of endodontic treatment and the smear layer reduces the efficacy of the apical seal. The aim of this study was to evaluate the effect of Nd:YAG laser irradiation on the apical seal of root canal-treated teeth using the fluid filtration technique.Materials And MethodsIn this in vitro study, 44 human maxillary central incisors were selected. After preparation, the specimens were randomly assigned to 4 groups: group A, the control group (n=20); group B, the laser-treated group (n=20); group C, the positive control group (n=2); and group D, the negative control group (n=2). All the groups were prepared using the crown-down technique. Group B was irradiated with Nd:YAG laser before obturation. Groups A, B and D were obturated with gutta-percha and sealer and group C was obturated with gutta-percha and without sealer. The coronal end of the specimens were sealed with amalgam and then stored in an incubator for one week. Then, the outer surface of groups A, B and C, except for 2 mm in the apical area, and group D were entirely covered with two layers of nail polish and one layer of sticky wax. Apical microleakage was then evaluated using fluid filtration technique. Data were analyzed with SPSS 18, using Kolmogorov-Smirnov and independent t-test (α=0.05).ResultsThe means of apical leakage in the laser and control groups were 4.35±2.88 and 5.34±3.13 µL/min/cm H2O, respectively, with no significant differences (p value = 0.305).ConclusionUnder the limitations of this study, use of Nd:YAG laser in root canal therapy had no significant effect on apical microleakage.Keywords: Leakage, Nd:YAG laser, Smear layer

Pages 239-247IntroductionSports activities are one of the main etiologic factors of traumatic dental injuries. The purpose of this study was to assess the level of knowledge in athletes and their coaches regarding management of dental traumas.Materials And MethodsThis cross-sectional study was performed on professional athletes of contact sports and their coaches in Mashhad using the census technique. Data were collected using a questionnaire which surveyed the athletes’ personal and occupational data and consisted of questions about the management of four dental traumas. Statistical analysis was carried out with chi-squared and Mann-Whitney tests (α=0.05).ResultsThe majority of athletes (44.2%) were 20‒30 years of age and the majority of coaches (44%) were over 40 years of age. Sports activity durations in the majority of athletes (37%) and coaches (36.6%) were 1‒5 and more than 10 years, respectively. The level of knowledge in the management of dental traumas in 3.8% of athletes was moderate; with 45% having low and 43.8% having very low knowledge levels and 7.5% had no awareness at all. The level of knowledge in 6.6% of coaches was high, with 17.1% having moderate, 53.9% having low, and 17.1% having very low knowledge levels; 3% had no knowledge at all. There was no significant correlation between level of knowledge and sex, age and sporting activity duration in athletes and coaches (p value > 0.05).ConclusionUnder the limitations of this study, the level of knowledge in athletes and coaches was insufficient in relation to dental traumas. Therefore, it appears it is necessary to prepare educational courses to improve the knowledge and awareness of athletes and coaches on the subject.Keywords: Athletes, Injuries, Knowledge

Pages 248-257IntroductionCross-infection control in dental offices is of great priority because of continuous contact with contaminated oral fluids and saliva. Therefore the aim of this study was to evaluate some principles of cross-infection control in dental health centers in Isfahan.Materials And MethodsThis cross-sectional study was carried out on 117 general dentists and specialists working in private dental offices and clinics in Isfahan in 2013‒2014. The subjects were selected using simple sampling technique. Data were collected with a questionnaire consisting of 18 questions on sterilization of instruments, disinfection of prostheses received from the laboratory, impressions and personal protection techniques during treatment procedures. Data were analyzed with SPSS 20 using descriptive statistics and Kruskal‒Wallis and chi-squared tests (α=0.05).ResultsUse of eyeglasses was more common in specialists (p value < 0.001). Specialists were more careful about disinfection of prostheses after checking them in the oral cavity before sending them to the laboratory compared to general practitioners (p value = 0.001). There was no significant difference between the two groups in rinsing impressions after retrieving them from the oral cavity (p value = 0.245). However, specialists paid more attention to disinfection of impressions after removing them from the patients’ oral cavities compared to general practitioners (p value = 0.002).ConclusionsUnder the limitations of the present study, it can be concluded that observation of the principles of cross-infection control was at a moderate level in dental treatment centers in Isfahan.Keywords: Infection control, Cross infection, Dentistry

Pages 258-265Background And AimPeriodontal diseases are one of the major problem in oral health. Periodontiis is the major cause of tooth loss in adults. The aim of this study was effect of periodontal treatments on Oral Health Quality of Life in patients with periodontal disease who referred to periodontology department in Kerman Dental school.Material And MethodsThis cross-sectional study was assessed on 45 patients with periodontal diseases. Data were collected by a questionnaire consisted of demographic status, site of surgery and Persian version of OHIP-14 questionnaire and periodontal status examination. Data analyzed in SPSS version 18 and using t-test and linear regression test. P value was considered at 5%level.ResultsIn this study 66.6% of subjects were female and mean age of samples were 38.8±6.67 years. The mean of OHIP-14 score before and after treatment were29.33±3.00 and 23.82 ±5.74 respectively, this differences was significant. There was significant differences between severity of periodontitis and OHIP-14 score before treatment (p=0/03). There was significant differences between severity of periodontitis and gender with OHIP-14 score after treatment (p=0.04 and 0.002).ConclusionIt seems periodontal diseases have negative impact on oral health related quality of life, and treatment can improved quality of life.Keywords: Oral Health, Quality of Life, Periodontitis
