اسپرم در مراحل مختلف اسپرماتوژنز نسبت به گونه های فعال اکسیژن (ROS) آسییب پذیر است. با این حال، سطح ROS سیمن توسط آنتی اکسیدان ها که اثرات مفیدی بر روی پارامترها و پتانسیل باروری اسپرم دارند، کنترل شده است. میتوکندری و غشای پلاسمایی اسپرم دو محل اصلی تولید ROS در این سلول ها می باشند. علاوه بر این، لکوسیت ها شامل پلی مورفونوکلویرها (PMN) و ماکروفاژها، تولید دسته گسترده ای از مولکول های اکسیژن از جمله رادیکال های آزاد، گونه های غیررادیکال و گونه های نیتروژن فعال را به عهده دارند. نقش فیزیولوژیکیROS ، افزایش cAMP داخل سلولی و پس از آن فعال سازی پروتئین کیناز در سیستم تناسلی مرد است که نشان می دهد اسپرم به مقدار کمی ازROS ، برای به دست آوردن توانایی و تنظیم بلوغ و تراکم هسته ای جهت بارور نمودن تخمک نیاز دارد. عوامل درونی و بیرونی متعددی است که می تواند در تعامل با لیپیدها و پروتئین به ایجاد استرس اکسیداتیو و تولید ROS بیانجامد که نتیجه آن، پراکسیداسیون لیپیدی، قطعه قطعه شدن DNA، آسیب آکسونم، دناتوره شدن آنزیم ها، افزایش یون سوپراکساید در میتوکندری، کاهش فعالیت آنتی اکسیدانی و در نهایت اختلال در روند اسپرماتوژنز خواهد بود. به طور کلی علل تولید استرس اکسیداتیو شامل طیف گسترده ای از عوامل بالینی و زیست محیطی می باشد. اگر استرس اکسیداتیو به عنوان عامل اصلی آسیب به DNA در اسپرم پذیرفته شود، پس باید دلیل خوبی برای درمان آنتی اکسیدانی در این شرایط وجود داشته باشد.

اندومتریوز تحت تاثیر دو عامل ژنتیک و محیط قرار دارد. ارتباط پلی مورفیسم های ژن های گلوتاتیون S-ترانسفراز (GSTs) با بیماری اندومتریوز مورد مطالعه پژوهشگران قرار گرفته است؛ با این وجود نتایج آن ها با هم همخوانی ندارد.
هدفدر این بررسی ما ارتباط بین ژنوتیپ های نول در ژن های GSTM1 و GSTT1 و همچنین GSTP1 313 A/G را با بیماری اندومتریوز در جمعیت ایرانی مورد مطالعه قرار دادیم.
مواد و روش هادر این بررسی مورد-شاهد در مجموع 151 زن مبتلا به اندومتریوز و 156 زن سالم به عنوان گروه کنترل وارد شدند. بررسی ژنوتیپ ها با استفاده از multiplex PCR و PCR-RFLP انجام شد.
نتایجفراوانی ژنوتیپ نول GSTM1 به طور معنی داری (0/027=p) در گروه مورد (7/3%) نسبت به گروه کنترل (1/3%) بالاتر بود. اختلاف معنی داری بین فراوانی ژنوتیپ های GSTT1 در بین گروهای مورد و شاهد مشاهده نشد. ژنوتیپ GSTP1 313 AG به طور معنی داری (0/048=p) در گروه مورد (1/33%) نسبت به شاهد (44/4%) پایین تر بود.
نتیجه گیرینتایج ما نشان داد که پلی مورفیسم های GSTM1 و GSTP1 با احتمال ابتلا به اندومتریوز در زنان ایرانی مرتبط می باشد.

بسیاری از بیماران سرطانی که تحت درمان با رادیوتراپی قرار می گیرند احتمال دارد دچار صدمات جدی به ذخیره تخمدان و عضلات ماتریکس رحم شوند. تعدادی از ان بیماران شاید برای بچه دار شدن به کلینیک های ناباروری مراجعه کنند و از روش های درمانی هورمونی برای القاء تخمک گذاری (ART) استفاده کنند.
هدفاین مطالعه به بررسی باروری و همچنین جنین های موش های ماده ای که تحت تزریق هورمون های تحریک تخمدان قرار گرفته اند و قبل از جفت گیری تحت تابش کل بدن با پرتوی گاما قرار گرفته اند پرداخته است.
مواد و روش هاموش های ماده به طور تصادفی به گروه های کنترل، و سه گروه I (پرتو)،گروه II (القاء تخمک گذاری) ،گروه III (پرتو و القاء تخمک گذاری) تقسیم شدند. در گروهی که تحریک تخمک گذاری با استفاده از هورمون داشتند به موش ها دوزهای متفاوت PMSG و HCG تزیق شد. در گروه پرتو نیز دوزی از پرتوی گامای کبالت که استفاده شد 2و4 گری بوده است. تعداد جنین های موش ها وهمچنین وزن، دور سر، تعداد جنین های سالم، تعداد جنین ها با آنومالی مشخص در بدن، تعداد جنین های سقط شده و متوقف شده در همه گروه ها بررسی و ثبت شد.
نتایجدر گروه I و II افزایش دوز پرتو و هورمون منجر به کاهش در تعداد جنین های زنده (45 در 2 گری در مقایسه با 29 در 4 گری در گروه پرتو) همچنین از 76 در گروه 10 واحد به 48 در گروه 15 واحد رسید. در گروه III دوز بالاتر هورمون در حضور 2 گری پرتو باعث افزایش تعداد جنین های سقط شده، شد. تعداد جنین های سقط شده در زمان استفاده از دوز 15 واحد از هورمون به 15 عدد رسید در حالی که همین مورد در زمان استفاده از دوز پایین تر هورمون 6 عدد ثبت شده است. در میان پارامترهای مختلفی که بررسی شد تفاوت مشخصی در پارامتر وزن جنین های موش مشاهده شد (0/01=p).
نتیجه گیرینتایج نشان می دهد که استفاده از هورمون های محرک تخمدان در موش هایی که قبل از جفت گیری تابش گاما دریافت کرده اند باعث افزایش قابل توجهی در شانس باردارشدن آن ها نمی شود.

لینولییک اسید فراونترین اسید چرب در مایع فولیکولی بوده که به عنوان ممانعت کننده بلوغ و رشد اووسیت شناخته شده است.
هدفدر مطالعه حاضر هدف بررس اثر لینولییک اسید روی اتساع سلول های کومولوس اووفروس، بلوغ اووسیت، رشد بلاستوسیست و بیان ژن های Bax و Bcl2 در بلاستوسیت گوسفند می باشد.
مواد و روش هابرای این منظور از 450 اووسیت که حداقل دارای 2 لایه یا بیشتر سلولول کوکولوس اطراف اووسیت بود، استفاده شد. مجموعه کمپلکس اووسیت- کومولوس به شکل تصادرفی در یکی از 4 گروه درمانی شامل گروه کنترل بدون لینولییک اسید و گروه های با غلظت های 50، 100 و 200 میکرومولار در هر میلی لیتر قرار گرفته و به مدت 24 ساعت کشت داده شد.
نتایجافزودن 200 میکرومولار در هر میلی لیتر لینولییک اسید به محیط کشت باعث کاهش اتساع سلولهای کومولوس اووفروس و تشکیل بلاستوسیت در مقایسه با گروه کنترل گردید. افزودن 100 میکرومولار در هر میلی لیتر لینولییک اسید به محیط بلوغ باعث افزایش اتساع سلول های کومولوس- اووفروس اطراف اووسیت و افزایش درصد رسیدن اووسیت ها به مرحله متافاز 2 در مقایسه با گروه کنترل شد. هیچ تفاوتی بین درصد تشکیل بلاستوسیست در غلظت های 50، 100 و 200 میکرومولار دیده نشد (13%، 14% و 12%).
نتیجه گیرینتایج این مطالعه نشان داد اضافه کردن لینولینولییک اسید به محیط بلوغ اووسیت ها شاید باعث بهبود میزان رشد بلاستوسیت ها در آزمایشگاه نشود.

مقاومت به انسولین در پاتوژنز سندروم تخمدان پلی کیستیک (PCOS) نقش مهمی دارد ولی راجع به وجود مقاومت به انسولین و عواقب آندوکرین و متابولیک آن در بیماران PCOS غیر چاق توافق نظر وجود ندارد.
هدفبررسی و مقایسه مقاومت به انسولین و اختلالات آندوکرین و متابولیک مختلف در زنان چاق و غیرچاق مبتلا به سندروم تخمدان پلی کیستیک.
مواد و روش هااین مطالعه مقطعی از سال 2007 تا 2010 برروی 115 بیمار PCOS در محدوده سنی 16 تا 45 سال انجام شد. وجود مقاومت به انسولین و پیامدهای آندوکرین و متابولیک آن در دو گروه مقایسه گردید. اطلاعات با نرم افزارSPSS ورژن 16 مورد تجزیه و تحلیل قرار گرفت. 05/0>p معنی دار در نظر گرفته شد. تفاوت معنی داری در وجود مقاومت به انسولین بین دو گروه مشاهده نشد (357/0=p). دور کمر (001/0>p)، نسبت دور کمر به دور باسن (001/0>p)، فشار خون سیستولی (001/0>p) و دیاستولی (001/0>p)، قند خون ناشتا (003/0=p)، سطح انسولین (011/0=p)، HOMA-IR (004/0=p)، کلسترول تام (001/0=p) و تری گلیسرید (001/0>p) همه بطور معنی داری در بیماران چاق بیشتر بود. تفاوت معنی داری در تستوسترون تام (643/0=p) و آندروستندیون (736004/0=p) بین دو گروه وجود نداشت در حالی که DHEAS بطور معنی داری در زنان غیر چاق بالاتر بود (018/0=p). کبد چرب در 3/31% و سندروم متابولیک در 4/39% بیماران چاق دیده شد ولی هیچ یک از این دو مورد در گروه غیرچاق دیده نشد.
نتیجه گیریدر مطالعه ما عواقب متابولیک چاقی و مقاومت به انسولین در زنان چاق شایعتر بود ولی فعالیت محور آدرنال بصورت افزایشDHEAS در زنان غیر چاق بطور شایعتری دیده شد .

شایع ترین بدخیمی در زنان سرطان پستان است. BRCA1 (ژن مستعدکننده سرطان سینه نوع 1) ژن سرکوبگر تومور است که در تعمیر آسیب های DNA نقش داشته و مسیول 81% موارد سرطان سینه و تخمدان خانوادگی می باشد. در برخی از ژن ها از نظر بالینی پلی مورفیسم مارکرهای داخل ژنی مهم است و غربالگری جهش از طریق پلی مورفیسم تکرارهای پشت سرهم کوتاه (STR) سریعتر انجام می پذیرد. پلی مورفیسم در یک STR ابزار خوبی برای تعقیب وراثت تکرار STR می باشد.
هدفهدف از این مطالعه بررسی پلی مورفیسم سه مارکر داخل ژنی BRCA1 در زنان دارای سرطان سینه با سابقه خانوادگی و مقایسه با زنان سالم بدون هیچ گونه سرطان و بیماری خاص می باشد.
مواد و روش هادر مجموع 107 بیمار با سرطان سینه و با سابقه خانوادگی سرطان و 93 زن سالم که هیچ فنوتیپ بالینی و بدخیمی نداشتند، گروه های مورد مطالعه را تشکیل می دهند. تجزیه و تحلیل هاپلوتیپ در 3 مارکر داخل ژنی BRCA1 (D17S855، D17S1322 و D17S1323)، در همه افراد بیمار و سالم با استفاده از پرایمرهای نشاندار صورت گرفت.
نتایجپس از تجزیه و تحلیل قطعات با روش فرگمنت آنالایز، با مقایسه با گروه سالم و بیمار، اختلاف قابل توجهی در دو آلل D17S855 مشاهده شد، آلل 146 (02/0=p) و 150 (006/0=p)، و همچنین این اختلاف در دو آلل D17S1322، آلل 121 (015/0=p) و 142 (043/0=p)، نیز مشاهده گردید. همچنین این اختلاف در هشت هاپلوتیپ دو یا سه آللی در بیماران وجومشاهده شد. برخی از هاپلوتیپها تا دو برابر خطر سرطان سینه را افزایش می دهند.
نتیجه گیریبا توجه به نتیجه اخیر ارزیابی حضور یا فقدان آلل های مذکور درمارکرهای داخل ژنی BRCA1 می تواند برای پیش آگهی ابتلای افراد مشکوک به سرطان سینه مورد استفاده قرار گیرد.

دیابت بارداری از جمله مشکلات رایج در طی حاملگی است که می تواند همراه با عوارضی در مادر و نوزاد باشد.
هدفاین مطالعه به منظور بررسی عوارض نوزادان متولد شده مادران مبتلا به GDM دریافت کننده انسولین در مقایسه با مادران دریافت کننده رژیم غذایی انجام شده است.
مواد و روش هادر این مطالعه مشاهده ای که به صورت یک کوهورت آینده نگر انجام شد، تعداد 140 نفر از نوزادان مادران مبتلا به GDM متولد شده در بیمارستان جواهری طی سالهای 1392 و 1393 وارد مطالعه شدند که 70 نفر از آنها انسولین و 70 نفر رژیم غذایی دریافت نمودند و میزان عوارض در نوزادان در دو گروه تعیین و مقایسه گردید.
نتایجنتایج به دست آمده در این مطالعه نشان دادند که %7/95 در گروه تحت رژیم غذایی و %7/85 در گروه دریافت کننده انسولین دارای نتایج بارداری خوب بودند که اختلاف آماری معناداری را نشان می داد (042/0=p) میزان مرگ و میری در دو گروه تفاوت آماری معناداری نداشت (05/0<p).
نتیجه گیریدر مجموع بر اساس نتایج این مطالعه چنین استنباط می شود که فراوانی عوارض در نوزادان متولد شده از مادران مبتلا به GDM دریافت کننده انسولین در مقایسه با مادران دریافت کننده رژیم غذایی بیشتر می باشد و لذا استفاده از رژیم غذایی تا حد امکان توصیه می شود.

آفالیا یا آژنزی پنیس یک مالفورماسیون نادر میباشد که با فقدان فالوس همراه است. این ناهنجاری بسیار نادر با آنومالی سیستم ادراری- تناسلی و پیامدهای روانی همراه است. شیوع آن 1 در 10-30 میلیون تولد تخمین زده شد.
موارد:
3 مورد از فقدان آلت تناسلی مردانه که با ناهنجاری های متعدد عضلانی اسکلتی، سیستم قلبی عروقی و سیستم اوروژنیتال همراه بودند مورد بررسی قرار گرفت.
نتیجه گیریآفالیا با پیامد روانی و پیش آگهی نسبتا خوب همراه است. این مطالعه نشان داد که اگر نارسایی کلیه که بدنبال انسداد ایجاد شود بر طرف گردد، این بیماران در شرایط مطلوب تر به دنیا می آیند و با توجه به تشخیص به موقع، عمل جراحی پیش از تولد و درناژ به موقع مثانه، اقدامات درمانی در آینده منوط به حفظ دستگاه تناسلی خارجی (مردانه) خواهد بود.

Sperm is particularly susceptible to reactive oxygen species (ROS) during critical phases of spermiogenesis. However, the level of seminal ROS is restricted by seminal antioxidants which have beneficial effects on sperm parameters and developmental potentials. Mitochondria and sperm plasma membrane are two major sites of ROS generation in sperm cells. Besides, leukocytes including polymer phonuclear (PMN) leukocytes and macrophages produce broad category of molecules including oxygen free radicals, non-radical species and reactive nitrogen species. Physiological role of ROS increase the intracellular cAMP which then activate protein kinase in male reproductive system. This indicates that spermatozoa need small amounts of ROS to acquire the ability of nuclear maturation regulation and condensation to fertilize the oocyte. There is a long list of intrinsic and extrinsic factors which can induce oxidative stress to interact with lipids, proteins and DNA molecules. As a result, we have lipid peroxidation, DNA fragmentation, axonemal damage, denaturation of the enzymes, over generation of superoxide in the mitochondria, lower antioxidant activity and finally abnormal spermatogenesis. If oxidative stress is considered as one of the main cause of DNA damage in the germ cells, then there should be good reason for antioxidant therapy in these conditions.

Endometriosis influenced by both genetic and environmental factors. Associations of glutathione S-transferases (GSTs) genes polymorphisms in endometriosis have been investigated by various researchers; however, the results are not consistent.
ObjectiveWe examined the associations of GSTM1 and GSTT1 null genotypes and GSTP1 313 A/G polymorphisms with endometriosis in an Iranian population.
Materials And MethodsIn this case-control study, 151 women with diagnosis of endometriosis and 156 normal healthy women as control group were included. The genotyping was determined using multiplex PCR and PCR- RFLP methods.
ResultsThe GSTM1 null genotype was significantly higher (p=0.027) in the cases (7.3%) than the control group (1.3%). There was no significant difference between the frequency of GSTT1 genotypes between the cases and controls. The GSTP1 313 AG genotype was significantly lower (p=0.048) in the case (33.1%) than the control group (44.4%).
ConclusionOur results showed that GSTM1 and GSTP1 polymorphisms may be associated with susceptibility of endometriosis in Iranian women.

Many cancer patients receive radiotherapy which may lead to serious damages to the ovary storage and the matrix muscle state. Some of these patients may admit to infertility clinics for having pregnancy and on the other hand hormonal administration for superovulation induction is a routine procedure in assisted reproduction technology (ART) clinics.
ObjectiveThis study aimed to investigate fertility and fetuses of hormone treated super ovulated female mice who had received whole-body gamma irradiation before mating.
Materials And MethodsFemale mice were randomly categorized into a control group and 3 experimental groups including: Group I (Irradiation), Group II (Superovulation), and Group III (Superovulation and Irradiation). In hormone treated groups, mice were injected with different doses of pregnant mare''s serum gonadotropin (PMSG) followed with human chorionic gonadotropin (HCG). Irradiation was done using a Co-60 gamma ray generator with doses of 2 and 4 Gy. Number of fetuses counted and the fetuss weight, head circumference, birth height, the number of live healthy fetuses, the number of fetuses with detected anomalies in the body, the sum of resorption and arrested fetuses were all recorded as outcome of treatments.
ResultsIn the group I and group II, increased radiation and hormone dose led to a decrease in the number of survived fetuses (45 in 2 Gy vs. 29 in 4 Gy for irradiated group) as well as from 76 in 10 units into 48 in 15 units. In the group III, a higher dose of hormone in the presence of a 2 Gy irradiation boosted the slink rate; i.e. the number of aborted fetuses reached 21 cases while applying the dose of 15 Iu, whereas 6 cases of abortion were reported applying the hormone with a lower dose. Among different parameters studied, there was a significant difference in parameters of weight and height in the mouse fetuses (p=0.01).
ConclusionThe data indicated that use of ovarian stimulating hormones in mice that received pre mating gamma irradiation did not significantly increase the pregnancy rates.

Materials And MethodsThe experiments conducted on 450 ovine Cumulus-oocyte complexes (COCs) with homogenous ooplasm and more than two compact layers of cumulus cells. For in vitro maturation COCs were randomly allocated into four treatment groups for 24 hr period. Treatment groups were as follow: control maturation media, 0 µM LA, 50 µM LA, 100 µM LA and 200 µM LA. The cumulus cell expansion and blastocysts rates were recorded. Total RNA was isolated from embryo pools, reverse transcribed into cDNA, and subjected to apoptotic gene expression by real-time PCR.
ResultsHighest concentration (200 µM/mL) of LA significantly decreased the rate of fully expanded cumulus cells 24 hr after in vitro maturation (IVM) and the percentage of blastocyste rate compared with the control (p
ConclusionData obtained in present study suggest that low concentration of LA used for maturation had no deleterious effect on subsequent embryonic development compared to high concentration of LA. Breast cancer is the most common malignancy in women. Breast Cancer Type 1 Susceptibility gene (BRCA1) is a tumor suppressor gene, involved in DNA damage repair and in 81% of the breast-ovarian cancer families were due to BRCA1. In some clinically investigated genes, the intragenic marker polymorphism is important and the screening of such mutations is faster by using short tandem repeat (STR) polymorphism. Individual polymorphism of STR is a good evidence for following inheritance of repeat polymorphism.
ObjectiveThe aim of this study was to evaluate three intragenic BRCA1 marker polymorphisms in families, which have two or more patients with breast/ovarian cancer in comparison to healthy women.
Materials And MethodsA total of 107 breast and/or ovarian cancer patients and 93 unrelated healthy women with no clinical phenotype of any malignancy or familial cancer history constitute the study groups. Haplotyping analysis, at 3 intragenic BRCA1 microsatellite markers (D17S855, D17S1322 and D17S1323), were performed for all subject and control groups using labeled primers.
ResultsAfter fragment analysis, significance differences were observed as follows: two alleles of D17S855; allele 146 (p=0.02) and 150 (p=0.006), and two alleles of D17S1322, allele 121 (p=0.015) and 142 (p=0.043). These differences were compared with control group. There was significance difference in 8 di/tri allelic haplotypes in present experimental subjects. Some haplotypes were observed to have approximately twice the relation risk for breast cancer.
ConclusionAccording to recent results, assessment of presence or absence of mentioned alleles in BRCA1 microsatellite can be used for prognosis in individuals, suspected of having or not having the breast cancer.

Materials And MethodsIn this prospective cohort study, 140 neonates of mothers with GDM attending Javaheri Hospital of Azad University in Tehran in 2013 and 2014 were enrolled and the complications in those receiving insulin versus. dietary regimen were compared.
ResultsThe results demonstrated that 95.7% of those who received a dietary regimen and 85.7% among those who received insulin had a good outcome showing statistically significant differences (p=0.042). The mortality rate was not differed among the patients in two groups (p>0.05).
ConclusionAccording to the results of this study, it may be concluded that the frequency of complications in neonates of cases with GDM is getting less by receiving dietary regimen.

Aphallia or penile agenesis is a rare malformation accompanying with no phallus. This anomaly is extremely rare with abnormality of urogenital system and psychological consequences. Its outbreak is estimated 1 out of 10-30 million births.
Case:
Reviewing 3 cases of male external genitalia agenesis, which associated with multiple anomalies of musculoskeletal, cardiovascular and genitourinary system.
ConclusionAphallia has psychosocial consequences and a guarded prognosis. This study showed that if the kidney failure is due to its obstruction, these patients will be born in more favorable conditions and the future treatment measures will be directed to keep the external genitalia (male) through timely diagnosis and prenatal surgery and timely bladder drainage.
