صفحات 400-405مقدمهدر این مطالعه، با استفاده از ترکیبی از روش های کاهش بعد داده و طبقه بندی، ویژگی های مواد دارویی مورد بررسی قرار گرفت. تعریف و آماده سازی مولکول های «غیر فعال» برای توسعه ی مدل های تفکیکی دوتایی (Two-class classifier) یکی از مشکلات عمده در مسیر استفاده از مدل های تفکیکی بر پایه ی لیگاند در روند طراحی سیستماتیک دارو می باشد. از این رو، با استفاده از مولکول های «فعال» موجود در پایگاه Binding-DB، به توسعه ی مدل های تفکیکی چند متغیره ی چند منظوره پرداخته شد.روش هابه این منظور، در حدود 160372 ریز مولکول برای 45 هدف دارویی مختلف از پایگاه مولکولی Binding-DB دانلود شد و پس از بهینه سازی ساختار، 1497 ویژگی فیزیکی و شیمیایی برای هر مولکول استخراج گردید. با استفاده از الگوریتم Apriori و ترکیب آن با روش طبقه بندی تفکیکی خطی (Linear discriminant analysis)، ویژگی های مولکولی برای هر هدف دارویی به منظور تفکیک مولکول های فعال استخراج شد.یافته هادر نهایت، با استفاده از غربالگری مجازی در پایگاه داده های مولکولی Zinc و Binding-DB و محاسبه ی سطح زیر نمودار Receiver operating characteristic (ROC) صحت و حساسیت طبقه بندی مورد بررسی قرار گرفت. میزان سطح زیر نمودار ROC برای هر بهینه سازی پایگاه Zinc به طور میانگین برابر با 1495/0 ± 8341/0 و در پایگاه Binding-DB به طور میانگین برابر با 1502/0 ± 8615/0 بود.نتیجه گیریمی توان با استفاده از الگوریتم ارایه شده، ویژگی هایی برای هر دسته از ریز مولکول های مرتبط با هر هدف دارویی استخراج کرد و پایگاه های مولکولی مختلف را برای هر هدف دارویی بهینه سازی کرد. سطح زیر نمودار ROC برای دو پایگاه مولکولی مورد بررسی نشان می دهد که روش ارایه شده، روش مفیدی برای طبقه بندی پایگاه های بزرگ مولکولی بدون استفاده از ریز مولکول های غیر فعال می باشد.کلیدواژگان: غربالگری مجازی، طبقه بندی چند منظوره، بررسی داده، طبقه بندی پایگاه داده، لیگاند

صفحات 406-411مقدمهیکی از مهم ترین اقدامات جهت نجات جان بیماران دچار Arrest قلبی، انجام عملیات احیا می باشد. این مطالعه، با هدف بررسی عملکرد تیم احیای بیمارستان چمران اصفهان انجام شد.روش هااین مطالعه ی مقطعی، شامل 211 نفر از بیمارانی بود که در سال 1394 در بیمارستان یا خارج از آن دچار Arrest قلبی- تنفسی شده بودند و برای آن ها، احیای قلبی- ریوی (Cardiopulmonary resuscitation یا CPR) شروع شده بود. اطلاعات احیای بیماران از روی چک لیست از پیش تهیه شده، تکمیل گردید.یافته هادر این مطالعه، 211 نفر بیمار شامل 140 نفر (35/66 درصد) مرد و 71 نفر (65/33 درصد) زن مورد بررسی قرار گرفتند. شاخص های دموگرافیک اعم از جنس، سن بیماران و محل شروع احیا (داخل و یا خارج بیمارستان) در موفقیت اولیه و بقای یک ماهه تاثیر نداشت (050/0 < P برای همه)، اما نیاز به احیای پیشرفته، فاصله ی زمانی بین Arrest تا شروع احیای پیشرفته، نیاز به شوک (در ریتم های قابل شوک)، فاصله ی زمانی Arrest تا اولین شوک، طول مدت CPR و انطباق احیای پایه و پیشرفته با الگوریتم American Heart Association (AHA) در موفقیت اولیه ی احیا تاثیر معنی داری داشت (050/0 > P برای همه)، اما در مورد بقای یک ماهه فقط فاصله ی زمانی Arrest تا اولین شوک تاثیر معنی داری داشت (010/0 = P).نتیجه گیریدر این مطالعه، تاثیر شاخص های دموگرافیک بر نتایج حاصل معنی دار نبود، اما نیاز به احیای پایه، پیشرفته و دریافت شوک در ریتم های قابل شوک و همچنین، فاصله تا انجام اولین شوک، تاثیر معنی داری بر موفقیت اولیه ی حاصل از احیا داشته است. عدم تطابق احیا در درصدی از موارد با راهنمای AHA، ضرورت بازآموزی منظم و مدون را گوشزد می کند.کلیدواژگان: احیای پایه، احیای پیشرفته، Arrest قلبی- تنفسی، احیای قلبی - ریوی

صفحات 412-421مقدمهشیوع چاقی کودکان در جهان رو به افزایش است. رژیم غذایی سالم، از جمله مداخلات مورد استفاده در درمان چاقی می باشد. روش های رفتاردرمانی می توانند بر رفتارهای غذایی کودکان موثر باشند. یکی از زیر شاخه های رفتاردرمانی، مصاحبه ی انگیزشی می باشد.روش هامطالعه ی حاضر، یک کارآزمایی بالینی تصادفی شده ی موازی بود. 96 کودک به طور تصادفی در سه گروه قرار گرفتند. گروه اول، رژیم درمانی و مصاحبه ی انگیزشی، گروه دوم رژیم درمانی و گروه سوم، گروه شاهد بودند. شرکت کنندگان 8 هفته تحت مداخله قرار گرفتند. شاخص های انتروپومتریک و فشار خون در ابتدا و انتهای مداخله سنجیده شد. جهت اطمینان از پایدار ماندن اثرات مداخله، افراد 3 ماه بعد بار دیگر مورد ارزیابی قرار گرفتند. میزان پای بندی به رژیم با محاسبه ی شاخص تغذیه ی سالم سنجیده شد.یافته هامیانگین نمایه ی توده ی بدنی و دور کمر در گروه رژیم درمانی و مصاحبه ی انگیزشی (001/0 > P) و در گروه رژیم درمانی (001/0 = P) بعد از مداخله، کاهش یافت، اما در گروه شاهد این اختلاف معنی دار نبود. این کاهش در گروه رژیم و مصاحبه ی انگیزشی نسبت به گروه رژیم بیشتر بود. بین میانگین فشار خون در سه زمان در هر سه گروه، اختلاف معنی داری مشاهده نشد. میانگین شاخص تغذیه ی سالم در گروه رژیم و مصاحبه ی انگیزشی نسبت به گروه رژیم بالاتر بود. بین شاخص تغذیه ی سالم با تغییرات نمایه ی توده ی بدنی و دور کمر، رابطه ی معکوسی وجود داشت (001/0 = P).نتیجه گیریرژیم غذایی سالم، منجر به کاهش نمایه ی توده ی بدنی و دور کمر کودکان چاق یا دچار اضافه وزن می گردد. همراه کردن رژیم درمانی با مصاحبه ی انگیزشی، تاثیر رژیم و میزان پای بندی کودکان به آن را افزایش می دهد.کلیدواژگان: مصاحبه ی انگیزشی، رژیم غذایی سالم، انتروپومتری، فشار خون، اضافه وزن

صفحات 422-427مقدمهPseudomonas aeruginosa یکی از مهم ترین عوامل عفونت های بیمارستانی به ویژه در بخش سوختگی با درصد مقاومت بالا در برابر دارو می باشد. یکی از مکانیسم های مقاومت در این باکتری، جهش های کروموزومی در ناحیه ی Quinolone-resistance-determining region (QRDR) ژن های کروموزومی می باشد. هدف از انجام این مطالعه، بررسی جهش ژن های gyrA و parC در جدایه های Pseudomonas aeruginosa مقاوم به سیپروفلوکساسین در نمونه های زخم سوختگی بود.روش هاتعداد 50 جدایه ی بالینی Pseudomonas aeruginosa جدا شده از بیماران بستری در بخش سوختگی بیمارستان سوانح سوختگی شهید صدوقی یزد تعیین هویت گردید. جهت تعیین حداقل غلظت مهار کنندگی (Minimum inhibitory concentration یا MIC) سیپروفلوکساسین از روش E-test و برای بررسی جهش در ژن های gyrA و parC در جدایه های مقاوم به سیپروفلوکساسین، از روش Polymerase chain reaction-sequencing (PCR-sequencing) استفاده شد.یافته ها62 درصد نمونه ها نسبت به آنتی بیوتیک سیپروفلوکساسین مقاوم بودند. جهش در gyrA در تمامی نمونه ها (100 درصد) و جهش در parC در 41 جدایه (82 درصد) از 50 جدایه دیده شد. فراوان ترین جهش در ژن gyrA تبدیل T83I و در ژن parC تبدیل S87L بود. هیچ گونه جهشی در جدایه های حساس یافت نشد.نتیجه گیریجهش در منطقه ی تعیین شده ی مقاومت کینولونی، یکی از مهم ترین مکانسیم های مقاومت به فلوروکینولون در جدایه های بالینی Pseudomonas aeruginosa می باشد. با توجه به شیوع این ژن ها، این جهش ها نقش مهم و اساسی را در ایجاد مقاومت ایفا می کنند.کلیدواژگان: Pseudomonas aeruginosa، زخم، سوختگی، فلوروکینولون ها

صفحات 428-433مقدمهیکی از بیماری های شایع در حیطه ی اورولوژی، سنگ های کلیه و مجاری ادراری است. بیماران پس از اعمال سنگ شکنی، دچار درد شدید پس از عمل می شوند که شاید یکی از علل آن، ناآگاهی از سیر درمان است. با توجه به اهمیت و کاربرد فراوان روش Percutaneous nephrolithotomy (PCNL) در درمان سنگ کلیه، مطالعه ی حاضر با هدف بررسی اثر آگاهی بخشی قبل از عمل بر شدت درد و اضطراب بیمار بعد از عمل انجام شد.روش هامطالعه ی حاضر از نوع کارآزمایی بالینی بود. شرکت کنندگان شامل 130 بیمار بودند که در Spiral computed tomography scan (Spiral CT scan)، وجود سنگ کلیه ی آن ها تایید شد. این افراد، به صورت تصادفی در دو گروه مورد و شاهد تقسیم شدند و اثر آموزش پیش از عمل بر درد و اضطراب بعد از عمل با استفاده از پرسش نامه های Visual analog scale (VAS) وBeck Anxiety Inventory (BAI) بررسی و با به کارگیری آزمون t مستقل، تجزیه و تحلیل شد.یافته ها130 بیمار شامل 65 خانم و 65 آقا وارد مطالعه شدند. میانگین نمره ی اضطراب در گروه شاهد 84/4 ± 89/34 و در گروه مورد 37/2 ± 98/22 بود (050/0 > P). همچنین، میانگین شدت درد در گروه شاهد 74/26 ± 44/74 و در گروه مورد 17/8 ± 18/34 بود (050/0 > P).نتیجه گیریدر مطالعه ی ما، میانگین شدت درد و اضطراب پس از عمل PCNL در بیمارانی که تحت آموزش قبل از عمل قرارگرفته اند، کاهش معنی داری را نشان می دهد. با توجه به هزینه های اندک آموزش بیماران پیش از عمل و تاثیرات بالقوه ی آن بر درد و اضطراب پس از عمل، این آموزش ها می تواند از نظر اقتصادی و همچنین، شرایط روحی بیمار تاثیر بالقوه ای داشته باشد.کلیدواژگان: سنگ کلیه، Percutaneous nephrolithotomy، مراقبت قبل از عمل جراحی

Pages 400-405BackgroundIn this research, we extracted and modified features of active ligands related to specific biological targets with combination of data mining and classification methods to aid medicinal chemists in their drug discovery projects. Preparing an inactive ligand is the major problem for development of multi-class classifiers. Therefore, our models were developed based on only active ligands found in Binding-database (DB) without any needs for preparing inactive molecules.MethodsOur database consisted of 160372 ligands in 45 classes of common proteins and 1497 different features (topological, chemistry, physical, etc.) were calculated for each molecule. Then, the specific features of active ligands of any target were extracted based on combination of linear discriminate analysis and Apriori algorithm.
Findings: Receiver operating characteristic (ROC) was a useful operator to analysis the accuracy and sensitivity of classification models and retrieving molecules from ZINC and Binding-DB databases. Area under curve (AUC) of this diagram was evaluated for analysis of each target in Zinc and Binding-DB and their results were 0.8341 ± 0.1495 and 0.8615 ± 0.1502, respectively.ConclusionSpecific features of active ligands could be found using the methodology described in this work and with these features, we can sort each database based on corresponding target. Pages 406-411BackgroundOne of the most important issues in order to save life in patients with cardiac arrest is cardiopulmonary resuscitation (CPR). This study evaluated the performance of resuscitation team in Chamran hospital, Isfahan, Iran, in 2015.MethodsThis cross-sectional study included 211 patients undergone CPR, in or out of the Chamran hospital. The CPR process data were collected from patient's files and the CPR heads filled out the checklists.
Findings: The patients were 140 men (66.35%) and 71 women (33.65%). The age, sex, and location of CPR had not significant effect on the outcome of CPR (P > 0.050 for all). Use of advanced life support, the time between cardiac arrest and start of advanced life support, the time between cardiac arrest and applying first shock, use of electroshock, CPR due to basic life support (BLS), CPR due to advanced life support (ALS), and duration of CPR had significant effect in primary survey of CPR (PConclusionIn our study, demographic indices did not have significant effect on primary CPR outcome but need to basic and advanced life supports, applying shock to shockable rhythm and also the time between cardiac arrest and applying first shock had significant effect on primary successful CPR. Incompatibility in the percentage of CPR cases with American Heart Association (AHA) guideline notes the necessity of regular and continuous retraining.Keywords: Advanced cardiac life support, Basic cardiac life support, Cardiopulmonary resuscitation (CPR) -
Pages 412-421BackgroundThe prevalence of children obesity is increasing all over the world. Healthy diet is one of the interventions for obesity treatment. Behavioral therapy approaches can affect childs food behaviors. One of these approaches is motivational interviewing.MethodsThis study was a paralleled randomized clinical trial. 96 children were randomly allocated to 3 groups: first group, diet and motivational interview, second group, diet, and the third one, control. Participants were under intervention for 8 weeks. Anthropometric indecies and blood pressure were measured at baseline and at the end of the intervention. To make sure about the maintenance of intervention effects, participates were assessed 3 month later again. Adherence to diet was assessed by measuring healthy eating index.
Findings: The mean body mass index (BMI) and waist circumference (WC) in first (PConclusionHealthy diet can reduce mean body mass index and waist circumference in overweight and obese children. Pages 422-427BackgroundPseudomonas aeruginosa is one of the most important factors for nosocomial infections with percentage of resistance to the drug, particularly in the burn wounds. One of the mechanisms of resistance in these bacteria is chromosomal mutation in the quinolone-resistance-determining region (QRDR) of chromosome gene. The aim of this study was to evaluate gyrA and parC gene mutation in Pseudomonas aeruginosa isolates from burn wound infections resistant to ciprofloxacin.Methods50 clinical Pseudomonas aeruginosa isolates were identified from patients admitted to burn hospital. Minimum inhibitory concentrations (MICs) of ciprofloxacin were evaluated by E-test method and polymerase chain reaction-sequencing (PCR-sequencing) method was carried out to assess the gyrA and parC mutations in ciprofloxacin-resistant isolates.
Findings: From 50 isolates, 62% were resistant to ciprofloxacin. Mutations were detected in all (100%) and 41 isolates (82%) in gyrA and parC genes, respectively. The most frequent mutations were observed in gyrA gene conversion (T83I) and parC (S87L). No mutation was found in sensitive isolates.ConclusionResults indicate that mutations in the quinolone-resistance-determining-region are the major mechanisms for ciprofloxacin resistance in clinical isolates of Pseudomonas aeruginosa. Pages 428-433BackgroundOne of the most common urologic diseases is renal and urinary tract stone. Patients experience severe pain after lithotripsy operation; one cause maybe the limited knowledge about the procedures. Due to important and considerable application of percutaneous nephrolithotomy (PCNL) procedure, we decided to evaluate the effect of preoperative education on postoperative pain and anxiety in these patients.MethodsThis clinical trial study included 130 patients who underwent the percutaneous nephrolithotomy. Nephrolithiasis was confirmed via spiral computed tomography (CT) scan in them. Patients divided randomly into two groups. This study followed with two parallel groups of equal size, and the effect of preoperative education on postoperative pain and anxiety evaluated by means of visual analog scale (VAS) and Beck Anxiety Inventory (BAI), respectively. Postoperative anxiety and pain intensity were compared using independent t test.
Findings: The mean anxiety level was 34.89 ± 4.84 and 22.98 ± 2.37 in control and case groups, respectively (PConclusionIn our study, postoperative stress and pain intensity after percutaneous nephrolithotomy were significantly less in experimental group.
