صفحات 293-308مقدمهنفروپاتی دیابتی یک بیماری مزمن کلیوی و یکی از شایع ترین عوارض دیابت نوع 2 است. مارکرهای کنونی تشخیصی نفروپاتی دیابتی مانند آلبومین و کراتینین تنها قادر به شناسایی این بیماری در مرحله آسیب کلیوی است. هدف از مطالعه حاضر بررسی متابولومیکس هدفمند اسیدهای آمینه سرم به منظور شناسایی ارتباط پروفایل اسیدهای آمینه سرم با دیابت و نفروپاتی دیابتی می باشد.روش هااین مطالعه مقطعی توصیفی- تحلیلی در سال 95-1394 بر روی 30 بیمار مبتلا به نفروپاتی دیابتی متعاقب دیابت نوع 2،30 بیمار مبتلا به دیابت نوع 2 مراجعه کننده به کلینیک دیابت 1 پژوهشگاه غدد و 30 فرد غیردیابتی انجام شد. هموگلوبین، HbA1c و BUN خون و سطح سرمی آلبومین،اسیداوریک، آلبومین نمونه رندوم ادرار صبحگاهی و کراتینین ادرار با روش های استاندارد وسطح سرمی اسیدهای آمینه با استفاده از کروماتوگرافی فاز معکوس با کارایی بالا (HPLC) اندازه گیری شد. از آزمون ANOVA و کروسکال والیس و از رگرسیون اسمی(Nominal regression) به منظور مقایسه گروه های مورد بررسی استفاده شد.یافته هاتفاوت آماری معنی داری در سطح سرمی هر 8 اسیدهای آمینه ضروری، اسیدهای آمینه شاخه دار، اسیدهای آمینه آروماتیک و نیز 8 اسیدهای آمینه غیرضروری آلانین، اسید آسپارتیک، سرین، گلوتامین، آرژینین، گلیسین، تیروزین و اورنی تین بین 3 گروه مورد بررسی نفروپاتی دیابتی، دیابت و افراد غیردیابتی مشاهده شد. سطح سرمی آرژینین و ایزولوسین در گروه دیابت بالاتر از غیردیابتی بود. در حالی که، مقادیر اسیدهای آمینه سرین، گلوتامین، گلیسین، ترئونین، تیروزین، تریپتوفان، متیونین، والین، اورنی تین و لیزین در 2 گروه نفروپاتی دیابتی و دیابت بالاتر از گروه غیردیابتی بود. به ازای هر انحراف معیار کاهش سطح سرمی اسیدهای آمینه سرین، آلانین و ایزولوسین در مقایسه با افراد دیابتی خطر نفروپاتی دیابتی به ترتیب 257/3 (فاصله اطمینان95%= 94/0- 10/0 و 039/0=P)، 207/2 (فاصله اطمینان95%= 81/0- 18/0 و 039/0=P) و 652/2 (فاصله اطمینان95%= 96/0- 21/0 و 012/0=P) برابر شد.نتیجه گیریکاهش سطوح سرمی اسیدهای آمینه سرین، لوسین و آلانین ممکن با بروز و پیشرفت نفروپاتی دیابتی مرتبط باشد و می توان از این ارتباط در اینده و با مطالعات بیشترکاربرد در کنترل متابولیک و بهبود پیش آگهی بیماران مبتلا به نفروپاتی دیابتی استفاده کرد.کلیدواژگان: نفروپاتی دیابتی، متابولومیکس هدفمند، اسیدهای آمینه، دیابت نوع 2

صفحات 309-316مقدمهبیماری دیابت علاوه بر مشکلات فیزیولوژیک، مشکلات روان شناختی نیز برای افراد به وجود می آورد. بنابراین هدف پژوهش تعیین اثربخشی درمان مبتنی بر پذیرش و تعهد بر سلامت روان بیماران مبتلا به دیابت نوع دو در شهر اصفهان بود.روش هاروش پژوهش نیمه آزمایشی و طرح آن به صورت پیش آزمون -پس آزمون با گروه گواه و مرحله ی پیگیری بود. جامعه ی آماری شامل بیماران دیابتی مرکز خیریه ی دیابت اصفهان در سال 1394 بود. جهت انجام پژوهش حاضر، از بین جامعه ی آماری تعداد 30 بیمار مبتلا به دیابت نوع دو، با استفاده از روش نمونه گیری غیر احتمالی در دسترس و هدفمند انتخاب و به صورت گمارش تصادفی در دو گروه آزمایش و گواه گمارده شدند (15 نفر در گروه آزمایش و 15 نفر در گروه گواه). ابزارهای مورد استفاده در این پژوهش پرسشنامه ی سلامت عمومی (GHQ) و پرسشنامه ی جمعیت شناسی بود. پس از اجرای پیش آزمون، گروه آزمایش درمان مبتنی بر پذیرش و تعهد را طی 8 جلسه 90 دقیقه ای در طی 8 هفته دریافت کردد، در حالی که گروه گواه در طی فرآیند انجام پژوهش، از دریافت این مداخله بی بهره بود. پس از تمام جلسات درمانی، افراد هر دو گروه تحت انجام پس آزمون قرار گرفتند. پس از دو ماه آزمون پیگیری نیز صورت پذیرفت. پس از جمع آوری داده ها، عمل تحلیل داده ها، به وسیله روش تحلیل آماری کواریانس با استفاده از بسته نرم افزاری SPSS21 صورت پذیرفت.یافته هانتایج نشان داد که بین نمرات پس آزمون و پیش آزمون گروه آزمایش در مقایسه با گروه کنترل، تفاوت معناداری وجود دارد(001/0P<). نتایج همچنین نشان داد اثر درمان در زمان پیگیری ماندگار بود (001/0P<).نتیجه گیریبا توجه به تاثیر معنادار درمان مبتنی بر پذیرش و تعهد بر سلامت روان بیماران مبتلا به دیابت نوع دو، پیشنهاد می شود از این روش در جهت افزایش سلامت روان این بیماران بهره برده شود.کلیدواژگان: سلامت روان، درمان مبتنی بر پذیرش و تعهد، دیابت نوع دو

صفحات 317-322مقدمهبیماری دیابت یک بیماری شایع و یکی از بزرگترین مشکلات بهداشتی کشور می باشد. این بیماران علاوه بر مسائل جسمی، ازمشکلات روحی- روانی نیز رنج می برد که اختلال در تصویر ذهنی از بدن یکی از برجسته ترین آن مشکلات است. مطالعه ی حاضر با هدف مقایسه ی تصویر ذهنی از خود در بیماران مبتلا به دیابت نوع دو تحت درمان با داروی خوراکی با بیماران تحت درمان با انسولین می باشد.روش هااین مطالعه از نوع توصیفی مقایسه ای است که به صورت مقطعی در سال 1394 با جامعه ی آماری شامل بیماران دیابتی نوع دو تحت پوشش مرکز دیابت شهرستان آران و بیدگل انجام شد. حجم نمونه 168 نفر، که از طریق نمونه گیری به روش تصادفی ساده انتخاب و از نظز سن وجنس همتا شده اند. داده های پژوهش با استفاده از پرسشنامه ی اطلاعات دموگرافیک و پرسشنامه ی چندبعدی نگرش فرد در مورد تصویر بدنی خود (MBSRQ) دارای پایای73/ جمع آوری گردیده و با استفاده از آزمون های آماری کای دو و t مستقل و فیشر و من ویتنی با نرم افزار SPSS 22 مورد تجزیه و تحلیل قرار گرفت.یافته هایافته های پژوهش حاکی از آنند که دو گروه از لحاظ توزیع جنسیت کاملا مشابه بوده و تفاوت آماری معناداری بین آن ها مشاهده نشد (05/0نتیجه گیریبه طور کلی میانگین تصویر ذهنی در بیماران تحت درمان با داروی خوراکی نسبت به بیماران تحت درمان با انسولین بیشتر نبود.از آنجا که یکی از تاثیرات منفی دیابت بر روی بیماران، اختلالات روانی می باشد لذا انتظار می رود جهت بهبود وضعیت روانی این بیماران و ارتقاء وضعیت سلامت و کنترل بیماری در این مبتلایان، از یافته های پژوهش پیش رو بتوان استفاده نمود.کلیدواژگان: دیابت، تصویر ذهنی از خود، داروی خوراکی دیابت، انسولین درمانی

صفحات 323-330مقدمهبا عنایت به ماهیت مزمن بیماری دیابت و تاثیر قابل ملاحظه ی آن بر کیفیت زندگی بیماران مبتلا، مطالعه ای با هدف بررسی کیفیت زندگی مرتبط با سلامت بیماران دیابتی نوع دوم مراجعه کننده به کلینیک دیابت سبزوار و عوامل مرتبط با آن به منظور برنامه ریزی مداخلات در حوزه ی ارتقای سلامت طراحی و مورد بررسی قرار گرفت.روش هادر این مطالعه توصیفی- مقطعی، 197 نفر از بیماران مبتلا به دیابت نوع دو مراجعه کننده به کلینیک دیابت به روش نمونه گیری در دسترس شرکت نمودند. داده ها از طریق پرسشنامه ی دو قسمتی شامل مشخصات فردی و پرسشنامه ی کیفیت زندگی SF36 با 8 مقیاس عملکرد جسمی، محدودیت جسمی، درد جسمی، سلامت عمومی، نشاط، عملکرد اجتماعی، مشکلات روحی، سلامت روان جمع آوری شد. تحلیل داده ها با استفاده از نرم افزار SPSS16 و آماره های توصیفی(فراوانی و درصد، میانگین و انحراف معیار) و آزمون های کای دو، آنالیز واریانس یک طرفه، تی تست و همبستگی در سطح معناداری 05/0 انجام شد.یافته هانتایج بیانگر آن است بالاترین نمره مربوط به عملکرد جسمی 38/22±61/74 و کمترین میانگین نمره را مقیاس محدودیت های ایفای نقش به علت مشکلات جسمانی 41/30±89/42 به خود اختصاص داده بود اما در کل، نمره ی میانگین سایر مقیاس ها در وضعیت «تا حدودی مطلوب» قرار داشتند. بین نمره ی عملکرد فیزیکی، احساس خوب بودن و درد برحسب تحصیلات افراد تفاوت آماری معنی دار وجود داشت (005/0 P ).نتیجه گیرییافته های این بررسی مارا در زمینه ی تاثیر بیماری دیابت بر کیفیت زندگی بیماران مبتلا به این بیماری آگاه ساخته که می توان از این اطلاعات در راستای برنامه ریزی درمانی مراقبتی بیماران مزمن و اصلاح کیفیت زندگی آنان بهره جست.کلیدواژگان: کیفیت زندگی، دیابت نوع دو، سلامت

صفحات 331-338مقدمهبه طور معمول مازاد تغذیه، در بدن به صورت چربی ذخیره و به چاقی منجر می شود. یکی از عوامل ابتلا به دیابت نوع دو، چاقی است. سایتوکاین های پیش التهابی مانند Tumor necrosis factorγ (TNF- α) و Interferon gamma (IFNγ) به پیام رسانی انسولین در بافت های حساس انسولین آسیب می زنند. در این مطالعه تاثیر عصاره سیر بر بیان سایتوکاین های IFNγ و TNF- α مترشحه از سلول های منونوکلئار اندازه گیری شد.روش هاپس از گرفتن خون محیطی بیماران مبتلابه دیابت نوع دو، سلول های منونوکلئار آن با روش فایکول جداسازی گردید و با دو غلظت عصاره سیر (30% و 10%)، برای مدت 48 و 72 ساعت کشت داده شد. سپس برای اندازه گیری میزان ترشح سایتوکاین IFNγ و TNF- α از روش الایزا استفاده شد.یافته هاعصاره ی سیر سبب کاهش میزان ترشح TNF-α از سلول های منونوکلئار شد هرچند که زمان و غلظت عصاره بر نتایج بی تاثیر بود همچنین عصاره ی سیر در میزان ترشح IFNγ نقشی نداشت.نتیجه گیریبا استفاده از عصاره ی سیر می توان استراتژی های درمانی را در پیش گرفت که هدف آن ها هم التهاب و هم مقاومت به انسولین بافت چربی است که برای پیشگیری و درمان دیابت نوع دو مفید است.کلیدواژگان: عصاره سیر، التهاب، دیابت نوع دو، TNF-?، IFN?

صفحات 339-348مقدمهادیپوکاین های امنتین-1، وسپین و پروتئین باند شونده به رتینول- 4 (4RBP-) میانجی های ترکیب بدن و بافت چربی هستند که در تغییرات متابولیکی ناشی از چاقی نقش دارند. این پژوهش با هدف تعیین رابطه بین سطوح سرمی این ادیپوکاین ها با اجزای ترکیب بدن و وضعیت متابولیکی در فنوتیپ های مختلف اندازه ی بدن صورت گرفت.روش هادر مطالعه ی مقطعی حاضر 350 زن شامل 127 نمونه با وضعیت متابولیکی سالم و 223 نمونه با وضعیت متابولیکی ناسالم شرکت کردند. در صورت وجود حداقل 4 مورد از پنج شاخص متابولیکی mmol/L70/1TG≤، mmol/L 60/2 LDL-C≤، mmol/L 20/5TC≤، HDL-C≥1/30 و 95/1HOMA-IR≤، نمونه ها دارای وضعیت متابولیکی سالم و در غیر این صورت دارای وضعیت متابولیکی ناسالم در نظر گرفته شدند.یافته هانتایج نشان داد که سطوح سرمی امنتین-1 با وضعیت متابولیکی سالم رابطه ی مثبت معنی داری دارد (0001/0p< ،04/1 OR=). حتی پس از کنترل عوامل مخدوشگر سن، جنس و وزن این ارتباط همچنان معنی دار بود. سطوح وسپین و RBP-4 با وضعیت متابولیکی سالم همبستگی معنی داری نداشت. همچنین، رابطه ی معنی داری بین امنتین و نمایه ی توده ی بدنی (02/0p=،31/0r=) و درصد چربی بدن (02/0p=،32/0r=) در گروه با وضعیت متابولیکی سالم مشاهده شد. در گروه سالم، سطوح سرمی RBP-4 با درصد چربی (0001/0p<،68/0r=) و توده ی چربی بدن (0001/0p<،74/0r=) همبستگی مثبت و با توده ی بدون چربی (0001/0p<،74/0- r=) و محتوای تام آب بدن (0001/0p<،74/0- r=) همبستگی منفی داشت. رابطه ی آماری معنی داری بین سطوح سرمی وسپین و شاخص های ترکیب بدن در هر دو گروه دیده نشد.نتیجه گیریسطوح سرمی امنتین-1 می تواند پیش بینی کننده ی مناسبی برای وضعیت متابولیکی سالم باشد.کلیدواژگان: آدیپوکاین، امنتین، وسپین، 4-RBP، وضعیت متابولیکی، ترکیب بدن

Pages 293-308BackgroundDiabetic nephropathy is a chronic kidney disease and of more common complications of type 2 diabetes mellitus. The current diagnostic markers of diabetic nephropathy, albumin and creatinine, are only able to catch the disease in the stage of renal damage. The aim of this study is evaluation of targeted metabolomics of serum amino acids to identify the association of the changes of serum amino acid profile with diabetes and diabetic nephropathy.MethodsThis cross-sectional study was conducted in 2015-2016 on thirty patients with type 2 diabetes subsequent diabetic nephropathy and thirty type 2 diabetic patients without nephropathy attending diabetes clinic of endocrinology and metabolism institute and thirty non diabetic persons. Blood hemoglobin, HbA1c and BUN and also, serum albumin, uric acid and the albumin/creatinine ratio from a random urine specimen were measured by standard methods and serum amino acids level were identified using high performance liquid chromatography (HPLC). Statistical analysis ANOVA, Kruskal-Wallis, and nominal regression were used for the comparison of the investigated groups.Resultssignificant differences were seen in serum levels of 8 essential, branched-chains, aromatic and 8 non-essential amino acids alanine, aspartic acid, serine, glutamine, arginine, glycine, tyrosine and ornithine between three groups. Serum levels of arginine and isoleucine were higher in the diabetic group than non-diabetics. However, Levels of amino acids serine, glutamine, glycine, threonine, tyrosine, tryptophan, methionine, valine, ornithine, and lysine in 2 groups of diabetic nephropathy and diabetes were higher than non-diabetic patients.
For every standard deviation decrease in serum levels of amino acids serine, alanine and isoleucine, in comparison to diabetic patients, the risk of diabetic nephropathy were increased 3.257 (95%CI: 0.10- 0.94, P=0.039), 2.207 (95%CI: 0.18- 0.81, P=0.039) and 2.652 (0.21- 0.96, P=0.012), respectively.ConclusionSince this study was conducted in patients in the early stages of the disease, reduced serum levels of the amino acids serine, leucine and alanine may be associated with development and progression of diabetic nephropathy. and in the future with more studies in this field can be used in metabolic control and improvement of the prognosis of patients with diabetic nephropathy.Keywords: Diabetic nephropathy_Targeted metabolomics_Aminoacids_Type 2 diabetes mellitus -
Pages 309-316Backgrounddiabetes causes psychological problems besides physiologic ones for the people. Therefore the purpose of the present study was to determine the effectiveness of acceptance and commitment therapy on mental health of the patients with type 2 diabetes in the city of Isfahan.Methodsthe research method was semi-experimental and its design was pretest, post-test with control group and follow-up stage. The statistical population of included diabetic patients in Isfahan charity diabetes center in 2015. In order to do the study, 30 patients with type 2 diabetes were selected through improbable available and purposeful sampling method and put into experimental and control groups (15 in the experimental and 15 in the control group). The applied instruments were General health questionnaire (GHQ) and demographic questionnaire. After administering pretest, the experimental group received acceptance and commitment therapy during eight ninety-minute sessions in eight weeks while the control group didnt have such intervention. After finishing therapeutic session, the people in two groups had the post test. After two months the follow-up test was administered. After data collection, the data analysis was conducted via COVARISNCE statistical analysis method through SPSS21 software.Resultsthe results showed that there is a significant difference between experimental and control group between pretest and posttest scores (PConclusionconsidering the significant effect of acceptance and commitment therapy on the mental health of the patients with type 2 diabetes, it is suggested to use this method to increase mental health of these patients.Keywords: Mental health_Acceptance_commitment therapy_Type 2 diabetes

Pages 317-322BackgroundDiabetes is a common disease and one of the biggest health problems in the country. These patients, in addition to physical problems, also suffered from mental problems which the disorder of body image is one of the most prominent problems. This study compared mental image of patients with type 2 diabetes treated with oral medication and those with insulin.MethodsThis study was conducted in a descriptive-analytic way in 1394. The study population included patients with type II diabetes are treated in diabetes center of Aran and Bidgol in 1394. A sample of 168 people was randomly chosen. Needed data was collected by demographic and multi-dimensional questionnaire using a person's attitude about their body image (MBSRQ) Dry reliable 0.73 and were analyzed with SPSS 22 software by using independent t tests, chi-square and Mann-Whitney and FisherResultsBase on the duration of disease, the most common was among patients with oral treatment (82.1 percent) and treated with insulin (58.3 per cent) of less than 10 years. Average rating of orientation to fit oral treatment was for patients with 41.79 and 40.15 for insulin therapy, in health assessment section for patients with 17.58 oral treatment and 16.71 for insulin. State of disease trends was in 63/1 percent of patients with moderate oral treatment and 46.4 percent with severe insulin treatment. Independent t test results showed a significant difference between the scores of two patients in the above sections (PConclusionGenerally, average mental image in patients treated with oral medication was not more than patients treated with insulin. Because one of the negative effects of diabetes on patients is psychiatric disorder, therefore, to improve the mental state of these patients and improve health and to control disease in these patients, research findings is expected to be the use.Keywords: Type 2 Diabetic_Self-Body Image_Oral Drug_Insulin Therapy

Pages 323-330BackgroundConsidering the importance of chronic diabetes and significant impact on patient's quality of life, A study to evaluate health-related quality of life in type II diabetic patients referred to diabetes clinic in Sabzevar And factors associated with planning and designing health promotion interventions in the areas studiedMethodsThis cross-sectional study in which 197 patients with type II diabetes referred to diabetes clinic participated. Of data through a two-part questionnaire that included demographic characteristics, social and Quality of Life Questionnaire, which has a five physical, psychological, social, economic and disease and treatment, collected and then using SPSS version 16 and using Chi-square tests, ANOVA, t-test and Spearman correlation coefficient were analyzed.ResultsThe findings of the study suggests that most participants in the study were in the age group aged 50 to 60, 77.2% female, 85.8% were married, 74.6 percent were housewives. Average quality of life in terms of physical functioning, role limitations due to emotional problems and emotional health and physical pain was good. There was a significant difference between the score of physical function, feeling well and pain according to education (P≤ 0/05).ConclusionThe findings of this study have informed us about the impact of diabetes on the quality of life of patients with this disease, which can be used to plan the care of chronic patients and improve their quality of life.Keywords: Quality of life, Type II diabetes, Health

Pages 331-338BackgroundUsually excess feeding in stored in the body as fat and leads to overweight. One of the causes of catching type 2 diabetes is obesity. Proinflammatory cytokines such as TNF- α and IFNγ can damage insulin signaling in insulin-sensitive tissues. In this study, the effect of garlic extract on the expression of TNF-α and IFNγ secreted by the mononuclear cells was measured.MethodsAfter taking peripheral blood from patient with type 2 diabetes, mononuclear cells were extracted by Ficoll Method. Cells were a culture with two different concentration of garlic extract (10% and 30%) for 48 and 24 hours. Then for measuring TNF-α and IFNγ release level, ELISA method was used.ResultsGarlic extract can reduce the amount of TNF-α secretion from cells mononuclear, although the timing and concentration of the extract had no effect on the results. Moreover, garlic extract had no effect on the secretion of IFNγ.ConclusionUsing garlic extract can be adopted therapeutic strategies aimed at adipose tissue inflammation and insulin resistance that is useful for the prevention and treatment of type 2 diabetes.Keywords: Garlic extracts, Diabetes mellitus type II, Inflammation, TNF-?, IFN?

Pages 339-348BackgroundThe adipokines omentin-1, vaspin, and retinol binding protein-4 (RBP-4) are mediators of body composition and adipose tissue that contribute to metabolic changes caused by obesity. The aim of this study was to determine the association between serum levels of these adipokines with body composition indices and metabolic status in different phenotypes of body size.MethodsIn the present cross-sectional study, a total of 350 women, including 127 metabolically healthy (MH) subjects and 223 metabolically unhealthy (MUH) subjects participated. The subjects were considered as MH in the presence of at least 4 of the five following metabolic indices: TG ≤ 1.70 mmol/L, LDL-C≤ 2.60 mmol /L, TC≤ 5.20 mmol /L, HDL-C≥1.30, and HOMA-IR≤ 1.95; otherwise subjects were considered as MUH.ResultsThe results showed that serum levels of omentin-1 have a significant positive association with MHS (OR = 1.04, pConclusionSerum levels of adipokines may be an appropriate predictor for metabolically healthy status.
