سرطان پستان شایع ترین سرطان زنان و دومین علت مرگ در کل جهان می باشد. مطالعه حاضر به منظور بررسی روند شناسایی تعداد موارد جدید سرطان پستان و پیش بینی وقوع آن در آینده با تحلیل سری زمانی انجام شد.
روش بررسیمطالعه حاضر یک بررسی مقطعی در طی سال های 1386 تا سال 1396 به منظور پیش بینی الگوی موارد جدید سرطان پستان با استفاده از سری زمانی مدل خود رگرسیون ادغام شده با میانگین متحرک (ARIMA) در استان قزوین است. برای نشان دادن روند و ارایه پیش بینی برای یک سال آینده سرطان از مدل سری های زمانی Box-Jenkins و روش های تشخیص و ارزیابی آن استفاده شد. جهت توصیف و برازش مدل های مناسب، از نرم افزار آماری R نسخه 3/6/3 استفاده شد.
یافته هابین سال های 1386 تا 1395 تعداد 1229 بیمار جدید ثبت شده است (میانگین (انحراف معیار) ماهانه 24/10(03/1) مورد). هرچند روند کلی تعداد خام موارد جدید سرطان پستان در طول زمان افزایشی می باشد ولی تغییر مشاهدات در طول زمان به صورت افزایشی و کاهشی بوده است بر اساس نتایج آزمون بارتلت، واریانس های داده های این سری زمانی سرطان ثابت نبودند. همچنین بر اساس نتایج آزمون کولموگروف-اسمیرنوف، داده های سری سرطان پستان نرمال نبودند. از بین مدل های بررسی شده، مدل (1، 1، 1) ARIMA به دلیل کمتر بودن معیار AIC نسبت به مدل های دیگر، انتخاب شد و این مدل به عنوان مدل نهایی برای پیش بینی سرطان پستان برای یک سال آینده انتخاب شد. فاصله اطمینان مقادیر پیش بینی شده نسبتا باریک بوده که مناسب بودن مدل نهایی را در پیش بینی نشان می دهد.
نتیجه گیریتحلیل سری زمانی ابزاری کارا جهت مدل سازی داده های گذشته و آینده تعداد خام موارد جدید سرطان پستان در استان قزوین می باشد. شاخص های نیکویی برازش مدل نشان داد مدل باکس جنکینز مدل قابل اتکایی برای برازش به داده های مشابه می باشد.

سرطان پستان یکی از شایع ترین سرطانهای زنان در سراسر جهان است. افراد مبتلا به سرطان ممکن است به دلیل پیشرفت بیماری یا به سایر دلایل بمیرند که به اینگونه از رخدادهای چندگانه ریسک های رقابتی میگویند. این مطالعه با هدف تعیین عوامل مرتبط با بقای بیماران مبتلا به سرطان پستان با استفاده از سه مدل رگرسیون مخاطرات علت- ویژه، زیرتوزیع و مدل انباشت تصادفی بقا انجام شده است.
روش بررسیمطالعه هم گروهی تاریخی بروی 527 بیماران سرطان پستان در همدان طی سالهای 1394-1383 انجام شد. برای تعیین عوامل خطر مرگ بهدلیل پیشرفت سرطان و مرگ بدون پیشرفت سرطان از مدلهای مخاطرات علت- ویژه، مخاطرات زیرتوزیع و انباشت تصادفی بقا در حضور ریسکهای رقابتی برازش داده شد. تحلیل داده ها با نرمافزار R3.4.3 انجام شد.
یافته هانتایج نشان داد که برای پیشامد مرگ ناشی از سرطان پستان، سن و تعداد غدد لنفاوی درگیر در هر دو مدل مخاطره علت- ویژه و زیرتوزیع معنادار است (05/0<p) و در مدل انباشت تصادفی با قاعده تقسیم لگ- رتبه و مدل انباشت تصادفی بقا با قاعده تقسیم Gray، برای پیشامد مرگ بر اثر سرطان متغیرهای سایز تومور، تعداد غدد لنفاوی درگیر، پروژسترون، استروژن و سابقه خانوادگی متغیرهای مهم شناسایی شدند.
نتیجه گیرینتیجه مدلهای مختلف در مطالعه حاضر نشان داد که تشخیص زودهنگام و مراجعات دورهای یکی از مهمترین عوامل برای جلوگیری از پیشرفت بیماری در این بیماران است. در حضور ریسکهای رقابتی در صورتی که فروض زیربنایی استفاده از مدلهای رگرسیون علت- ویژه و زیرتوزیع برقرار نباشد استفاده از مدل انباشت تصادفی بقا برای دادههای رخدادهای رقابتی بهمنظور تعیین عوامل خطر موثر بر بقا با توجه به شاخص هماهنگی و شاخص نمره بریر مناسبتر است. همچنین آنالیز بقا متوسط زمان پیگیری 3/49 ماه بود.

سرطان پستان از شایعترین سرطان های زنان بوده و ماستکتومی به عنوان روش جراحی در درمان سرطان پستان بر جنبه های مختلف زندگی بیماران اثرگذار است. هدف از این پژوهش تبیین تجربه زیسته زنان از زنانگی بعد از ماستکتومی و درمان بود.
روش بررسیاین مطالعه به روش کیفی بر روی 17 نفر از بیماران ماستکتومی شدهای که حداقل مدت زمان 6 ماه از ماستکتومی آنها گذشته بود و کمتر از 50 سال سن داشتند، با استفاده از مصاحبه نیمه ساختار یافته عمیق با روش نمونه گیری گلوله برفی در تهران در سال 1397 انجام شد. داده های بهدست آمده به روش تحلیل محتوای قراردادی مورد تجزیه و تحلیل قرار گرفتند.
یافته هاتحلیل داده های حاصل از تجارب بیماران منجربه شکلگیری 18 طبقه و 7 تم اصلی تحت عناوین: زندگی اجتماعی، شامل سه طبقه: کم شدن رفت و آمدها، ترک شغل یا کار نیمه وقت و ترک تحصیل. تصویر بدنی شامل سه طبقه: نامتقارن شدن، جذاب نبودن و نیاز به ترمیم عضو. تصور دیگران شامل دو طبقه: شکایت از نگاهه ا و کنجکاوی دیگران و شکایت از ترحم و دلسوزی دیگران. روابط زناشویی شامل چهار طبقه: ترس از ترک شدن، ترس از ترحم همسر، شرم از برهنگی، کاهش و قطع مقاربت. سلامت باروری (مسایل مرتبط با زنان و مامایی) شامل دو طبقه: یایسگی زودرس و نازایی احتمالی. ادراک حمایت شامل دو طبقه: شرکت در کلاس ها و همایش های خیریه و ارتباط با بهبودیافتگان. ایفای وظایف خانگی شامل دو طبقه: ناتوانی در کدبانوگری و ناتوانی در انجام کارهای دستی خانگی.
نتیجه گیریبر اساس یافته های مطالعه حاضر می توان نتیجه گرفت ماستکتومی و درمان سرطان پستان بسیاری از حوزه های زندگی فردی و اجتماعی و روابط زناشویی بیماران را به شدت تحت تاثیر قرار می دهد و این موضوع، ضرورت طراحی و استقرار منابع حمایتی رایگان از سوی سازمان های مردم نهاد و دولت را برای کاهش آسیب های روانشناختی و خانوادگی ناشی از این بیماری برجسته می سازد.

زیرگروه Her2-enriched در سرطان پستان پیش آگهی بدتری نسبت به زیرگروه های لومینال دارد. و اخیرا کشف درمان های هدفمند در سایر گروه های سرطان پستان منجر به افزایش بقاء بیماران شده است. هدف این مطالعه شناخت ژن های موثر در بقای کلی این دسته از بیماران مبتنی بر زیست شناسی سامانه ای است.
روش بررسیمجموعه داده های بیان ژن و اطلاعات بالینی 58 بیمار مبتلا به زیرگروه Her2-enriched از اطلس ژنوم سرطان دانلود شد. با استفاده از شبکه هم بیان ژنی وزن دار، ماژول های هم بیان شناسایی شدند. به منظور شناسایی ماژول های موثر بر بقای کلی، از رگرسیون کاکس استفاده شد. آنالیز بقاء تک ژنی در درون ماژول منتخب انجام و در پایان ژن های معنی دار با استفاده از ابزار DAVID مورد تفسیر عملکردی قرار گرفتند.
یافته هااز میان شش ماژول هم بیان شناسایی شده، دو ماژول به طور معنی دار با پیش آگهی بدتر در بیماران مرتبط بودند. بر مبنای آنالیز بقای تک ژنی، 39% از ژن های ماژول منتخب، معنادار شناخته شد که رابطه معناداری با مسیرهای زیستی "Ubiquitin mediated proteolysis" و "RNA degradation" داشتند. ژن های CHAMP1، PPP1R26، PRRC2B، KANSL3 و ANAPC2 به ترتیب به عنوان 5 ژن مهم در کاهش بقای کلی شناسایی شدند. همچنین این ژن ها در مسیرهای مرتبط با تقسیم سلولی نقش دارند که تاییدکننده نقش آنها در سرطان است.
نتیجه گیریرویکرد زیست شناسی سامانه ای می تواند نتایج مناسبی در تحلیل بقاء بیماران داشته باشد. در این مطالعه ژن هایی شناسایی شدند که می توانند در مطالعات آزمایشگاهی به عنوان بیومارکرهای پیش آگهی دهنده بقای کلی بیماران زیرگروه Her2-enriched و همچنین کاندیداهای بالقوه برای درمان هدفمند این دسته از بیماران مورد استفاده قرار گیرند.

استفاده از داروهای شیمی درمانی دارای عوارض جانبی و استفاده از ترکیبات گیاهی در درمان سرطان با چالشهایی روبرو است. نانوذرات و بهویژه لیپوزوم ها با داشتن ویژگی های مناسب در رسانش دارو از جمله رهایش آهسته دارو، سمیت پایین در محل سلول های هدف، می تواند بخشی از این مشکلات را برطرف نماید. بنابراین هدف از مطالعه حاضر، ساخت سامانه لیپوزومال حاوی عصاره Artemisia Absinthium و بررسی سمیت آن بر روی رده سلولMCF-7 است.
روش بررسیسه سامانه لیپوزومی حاوی عصاره با درصد مولی متفاوتی از SPC80 و SPC60 به همراه کلسترول بروش فیلم نازک تهیه گردید. فرمولاسیون مناسبتر با توجه به درصد بارگذاری و میزان رهایش انتخاب گردید. الگوی رهایش عصاره از فرمولاسیون منتخب، اندازه ذرات و پتانسیل زتا بررسی گردید. در پایان سمیت سامانه منتخب حاوی عصاره، و عصاره آزاد و سامانه فاقد عصاره بر رده سلولی MCF-7 سنجیده شد.
یافته هامیزان بارگذاری عصاره، اندازه ذرات، شاخص پراکندگی، و پتانسیل زتا برای فرمولاسیون منتخب، به ترتیب %4/0 +_ 2/49، nm9/121، 129/0 و mV6/10- می باشد. سامانه منتخب در شرایط دمایی مشابه سلول سالم و سرطانی، آهسته رهش بود. همچنین عصاره لیپوزومه نسبت به عصاره آزاد سمیت بیشتری بر رده سلولی MCF-7 داشت. و لیپوزوم های فاقد عصاره از سمیت ناچیزی بر رده سلولی MCF-7 برخوردار است.
نتیجه گیریسامانه منتخب لیپوزومی حاوی عصاره Artemisia Absinthium از سمیت بالاتری نسبت به عصاره آزاد بر سلول های سرطانی پستان برخوردار است که می تواند ناشی از ویژگی های فیزیوشیمیایی مناسب سامانه از جمله رهایش هدفمند باشد.

سرطان پستان مهمترین علت مرگ و میر ناشی از سرطان در زنان است که بررسی عوامل ایجاد کننده یا تشدید کننده آن ضروری به نظر می رسد. در این مطالعه، سطح mRNA ژن PHB2 در بافت تومور و بافت غیرتوموری مجاور مربوط به 50 زن مبتلا به کارسینوم داکتال غیرتهاجمی پستان مورد بررسی قرار گرفت.
روش بررسیمحلول RNX-Puls برای استخراج RNA کل از بافت توموری و غیرتوموری مجاور بیماران مبتلا به سرطان پستان استفاده شد. سپس با استفاده از کیتPrimeScriptTM RT، RNA کل به cDNA تبدیل شد. سطح mRNA ژن PHB2 توسط qRT-PCR مورد بررسی کمی قرار گرفته و داده ها با آزمون تی زوجی مورد آنالیز قرار گرفت. علاوه بر این، بررسی منحنی ROC برای ارزیابی پتانسیل بیومارکری PHB2 در بافت های توموری سرطان پستان انجام شد.
یافته هاسطح پایینی از PHB2 mRNA در بافت توموری بیماران مبتلا به سرطان پستان در مقایسه با بافت غیرتوموری مجاور مشاهده شد (P<0.0001). به عبارت دیگر، سطح PHB2 mRNA در بافت توموری نسبت به حالت نرمال تقریبا 5/2 برابر کاهش داشته است (P=0.001). آنالیز منحنی ROC نشان داد که سطح mRNA ژن PHB2 می تواند بافت های توموری و غیرتوموری را با اختصاصیت 3/91% و حساسیت 3/63% تشخیص دهد (AUC=0.712, P<0.001).
نتیجه گیریکاهش سطح mRNA ژن PHB2 در بافت توموری بیماران مبتلا به سرطان پستان نشان می دهد که سطح mRNA این ژن احتمالا بافت توموری را از بافت های غیرتوموری مجاور می تواند تمایز دهد.

Breast cancer is the most common cancer and the second leading cause of death in women worldwide. The aim of this study was to analyze the trend and predict the incidence of breast cancer using time series analysis.
MethodsIn this study, data on breast cancer incidence in Qazvin province between 2007 and 2016 were analyzed using time series analysis with autoregressive integrated moving average (ARIMA) modeling to forecast the future pattern. The Box-Jenkins time series model and its diagnosis and evaluation methods were used to show the trend and forecasting the next year new cancers. To describe and fit the appropriate models, R statistical software version 3.6.3 was used.
ResultsBetween 2007 and 2016, a total number of 1229 new patients had been registered (monthly mean [SD]: 10.24 [1.03]). Although the overall trend in the raw number of new breast cancer cases has been increasing over time, the change in observations over time has been increasing and decreasing. According to Bartlett test results, the variances of the data were not constant. Also, according to the results of Kolmogorov-Smirnov test, breast cancer series data were not normal. Among the studied models, ARIMA (1, 1, 1) was selected due to lower AIC criteria than other models, and this model was selected as the final model for predicting breast cancer for the next year. The confidence interval of the predicted values was relatively narrow, which indicates the appropriateness of the final model in the prediction.
ConclusionTime series analysis is an efficient tool to model the past and future data on the raw number of new cancer cases, and the goodness-of-fit indicators of the model showed that the Box-Jenkins model is a reliable model for fitting similar data.

Breast cancer is one of the most common cancers among women worldwide. Patients with cancer may die due to disease progression or other types of events. These different event types are called competing risks. This study aimed to determine the factors affecting the survival of patients with breast cancer using three different approaches: cause-specific hazards regression, subdistribution hazards regression, and the random survival forest for competing risks.
MethodsA historical cohort study was conducted on 527 breast cancer patients diagnosed in Mahdieh Medical Center, Hamadan, between 2004 and 2015. To determine risk factors for death due to cancer progression or other competing risks, cause-specific hazards and substandard hazards models and a random survival forest for competing risk were fitted. Data analysis was performed with R 3.4.3.
ResultsFindings showed that for death from the progression of breast cancer, age and number of involved lymph nodes were significant in both models (P < 0.05), and in the random survival forest model for death due to cancer progression, tumor size, number of involved lymph nodes, progesterone, estrogen, and family history were the important identified variables.
ConclusionIn the presence of competing risks, when the underlying assumptions of cause-specific and subdistribution hazard regression models are not established, the use of random survival forest for competing events data to determine the risk factors affecting survival according to the coordination index and Brier score is more appropriate.

Breast cancer is the most prevalent cancer among women, and mastectomy, the surgical treatment for this cancer, affects different aspects of patients’ life. This study aimed to describe women’s lived experiences of femininity after mastectomy and treatment.
MethodsThis qualitative study was conducted on 17 breast cancer survivors in Tehran in 2018-2019. The participants, who were selected through snowball sampling, had undergone mastectomy at least 6 months before and were younger than 50 years. Data were collected using in-depth semi-structured interviews and analyzed with the contractual content analysis method.
ResultsThe analysis of patients’ experiences led to the identification of 7 main themes (and 18 subtheme): social life (social isolation, quitting the job or getting a part-time job, and dropping out of school), body image (being asymmetric, being unattractive, and requiring reconstructive surgery), others’ opinions (complaining about how others look at them and concerns of being pitied), marital relationships (fear of abandonment, fear of spouse’s pity, shame of being nude, and compromised sexual intimacy), reproductive health (premature menopause and possible infertility), perceived support (attending support group meetings and keeping in touch with cured ones), taking responsibilities of household works (inability to do housewife affairs and inability to do the housework).
ConclusionMastectomy and treatment for breast cancer greatly affect many areas of individual and social life and marital relationships of patients, necessitating the need to develop and establish free support resources by NGOs and highlighting the government’s role in reducing the psychological and family damage caused by the disease.

HER2-enriched subtype of breast cancer has a worse prognosis than luminal subtypes. Recently, the discovery of targeted therapies in other groups of breast cancer has increased patient survival. The aim of this study was to identify genes that affect the overall survival of this group of patients based on a systems biology approach.
MethodsGene expression data and clinical information on 58 patients with HER2-enriched cancer were downloaded from The Cancer Genome Atlas (TCGA). Co-expression modules were identified using the weighted gene co-expression network analysis (WGCNA). The Cox regression was used to determine the modules that had a significant relationship with the overall survival (OS) endpoint. Single-gene survival analysis was performed within the selected module. Finally, functional annotation to explore the significance of genes was performed using the Database for Annotation, Visualization, and Integrated Discovery (DAVID).
ResultsOf the six identified co-expression modules, two had significantly poor prognoses. Single-gene survival analysis showed that 39% of genes in the selected modules were identified as significant. The genes were mainly related to the biological pathways such as Ubiquitin-mediated proteolysis and RNA degradation. CHAMP1, PPP1R26, PRRC2B, KANSL3, and ANAPC2 were identified as the 5 most important genes associated with reduced OS, in order of significance.
ConclusionThe systems biology approach can provide appropriate results relate to patient survival analysis. In this study, some genes were identified to be used as prognostic biomarkers in experimental studies related to the OS in the HER2-enriched subgroup. These genes can be considered potential candidates for therapeutic targets in this group of patients.

The use of chemotherapy drugs has side effects and the use of herbal compounds in the treatment of cancer faces challenges. Nanoparticles, especially liposomes, with appropriate properties in drug delivery, such as slow drug release, low toxicity at the target cell site, can solve some of these problems. Therefore, the aim of the present study was to construct a liposomal system containing Artemisia absintium extract and to investigate its toxicity on MCF-7 cell line.
MethodsThree liposomal systems containing extracts with different molar percentages of SPC80 and SPC60 along with cholesterol were prepared by thin film method.The most suitable formulation was selected according to the loading percentage and release rate. The release pattern of the extract from the selected formulation, particle size and zeta potential were investigated. Finally, the toxicity of the selected system containing the extract, the free extract and the non-extracted system on the MCF-7 cell line was measured.
ResultsExtract load, particle size, dispersion index, and zeta potential for the selected formulation are 49.2 ± 0.4%, 121.9 nm, 0.129 and -10.6 mV, respectively. Selected system at similar temperatures to healthy and cancer cells was slow to release. Also, liposome extract was more toxic to MCF-7 cell line than free extract and extract-free liposomes have little toxicity on MCF-7 cell line.
ConclusionThe selected liposomal system containing Artemisia absintium extract has a higher toxicity than the free extract on breast cancer cells, which can be due to the appropriate physicochemical properties of the system, including targeted release.

Breast cancer is the most important cause of cancer mortality among women, therefore the study of its causative or aggravating factors seems necessary. In this study, the mRNA levels of the PHB2 gene were evaluated in tumor and adjacent non-tumor tissues of 50 women diagnosed with invasive ductal carcinoma of the breast.
MethodsRNX-Plus solution was used to isolate total RNA from tumor and adjacent non-tumor tissues of breast cancer patients. Thereafter, total RNA was converted to cDNA using PrimeScriptTM RT reagent kit. The mRNA levels of PHB2 were quantified by qRT-PCR and data were analyzed with a paired-samples t test. Furthermore, ROC curve analysis was performed to evaluate the diagnostic capacity of PHB2 expression in breast cancer tumor tissues.
ResultsA significantly lower level of PHB2 mRNA was observed in tumor tissues of breast cancer patients compared with adjacent non-tumor tissues (P<0.0001). PHB2 mRNA levels showed an approximately 2.5-fold reduction in tumor tissues compared with normal tissues (P = 0.001). Roc cure analysis showed that PHB2 mRNA level can discriminate tumors from non-tumor tissues with 91.3% specificity and 64.3% sensitivity (AUC=0.712, P<0.001).
ConclusionDownregulation of PHB2 in breast cancer patients shows that its mRNA levels can possibly discriminate tumors from non-tumor tissues.
