باتوجه به تاثیر منفی شیفت های شبانه و طولانی مدت بر عملکرد حرفه ای پزشکان، پژوهش حاضر با هدف تعیین ارتباط بین طول مدت شیفت با عملکرد شناختی و کیفیت زندگی متخصصان طب اورژانس انجام شد.
روش هادر این مطالعه ی مقطعی، 25 نفر از اعضای هیات علمی، دستیاران و متخصصان طب اورژانس شرکت نمودند و عملکرد شناختی آنان با استفاده از نسخه ی چهارم مقیاس هوش بزرگسالان Wechsler (Wechsler Adult Intelligence Scale-Fourth Edition یا WAIS-IV) در پایان یک شیفت صبح 12 ساعته، یک شیفت شب 12 ساعته و یک شیفت طولانی مدت 24 ساعته بررسی گردید و بین سه شیفت مقایسه صورت گرفت. کیفیت زندگی شرکت کنندگان نیز با استفاده از فرم کوتاه شده ی (26 سوالی) پرسش نامه ی کیفیت زندگی سازمان جهانی بهداشت (World Health Organization Quality of Life-BREF یا WHOQOL-BREF) بررسی شد و ارتباط بین عملکرد شناختی و کیفیت زندگی آنان مورد ارزیابی قرار گرفت.
یافته هامیانگین نمره ی عملکرد شناختی در پایان شیفت صبح، شب و 24 ساعته به ترتیب 58/3 ± 80/73، 00/4 ± 24/69 و 01/4 ± 80/59 گزارش گردید و اختلاف بین سه شیفت معنی دار بود. میانگین نمره ی کیفیت زندگی شرکت کنندگان، 29/10 ± 52/80 به دست آمد. بین نمره ی کیفیت زندگی و عملکرد شناختی در شیفت 24 ساعته، همبستگی معکوس و معنی داری به میزان 386/0 مشاهده گردید.
نتیجه گیریعملکرد شناختی متخصصان طب اورژانس در پایان شیفت های 24 ساعته نسبت به شیفت های صبح و شب، کاهش معنی داری را نشان داد. از طرف دیگر، بین نمره ی اختلال عملکرد شناختی در پایان شیفت های 24 ساعته و کیفیت زندگی، ارتباط معنی داری وجود دارد.

هدف از انجام پژوهش حاضر، بررسی و مقایسه ی اثربخشی دو تکنیک تزریق خون اتولوگ و Dry needling در بیماران مبتلا به بیماری لترال اپی کندیلیت (سندرم آرنج تنیس بازان) مقاوم به درمان بود.
روش هااین مطالعه به صورت کارآزمایی بالینی تصادفی شده، بر روی 32 بیمار مبتلا به بیماری لترال اپی کندیلیت مقاوم به درمان انجام گرفت. بیماران پس از ورود به تحقیق، به دو گروه درمانی طبقه بندی شدند. شرکت کنندگان گروه اول و دوم به ترتیب تحت درمان با روش های Dry needling و تزریق خون اتولوگ قرار گرفتند. میزان درد بیماران با استفاده از مقیاس های Visual analogue scale (VAS) و Patient-Rated Tennis Elbow Evaluation (PRTEE) قبل از انجام مداخلات، 1 ماه بعد و 3 ماه پس از مداخله اندازه گیری و مقایسه گردید.
یافته هامیزان درد در هر دو گروه تزریق خون اتولوگ و Dry needling، یک ماه و سه ماه پس از درمان بر اساس هر دو معیار، کاهش معنی داری را نشان داد. بین دو گروه تزریق خون اتولوگ و Dry needling تفاوت معنی داری در کاهش میزان درد در طول زمان بر اساس معیارهای VAS وPRTEE مشاهده نشد.
بحثمیانگین نمره ی درد بیماران دو گروه از نظر سنجش با معیارهای VAS و PRTEE، تفاوت معنی داری نداشت. پس از انجام مداخلات، نتایج به دست آمده نشان دهنده ی کاهش معنی دار نمره ی درد در هر دو گروه نسبت به ابتدای پژوهش بود.

سویه های استافیلوکوکوس اوریوس مقاوم به متی سیلین (Methicillin-resistant Staphylococcus aureus یا MRSA)، به دلیل مقاومت در برابر بیشتر آنتی بیوتیک ها، از مشکلات بهداشتی در جهان محسوب می شود. پنتون والنتین لکوسیدین (Panton-Valentine leucocidin یا PVL) از جمله سموم مهم استافیلوکوکوس اوریوس می باشد. پژوهش حاضر با هدف تعیین فراوانی مقاومت به متی سیلین و جستجوی ژن های mecA و pvl در ایزوله های استافیلوکوکوس اوریوس جدا شده از نمونه های بالینی بیمارستان ولی عصر (عج) تبریز انجام شد.
روش هادر این مطالعه ی توصیفی- مقطعی که طی سال 99-1398 صورت گرفت، 142 جدایه ی استافیلوکوکوس اوریوس از بیماران بستری شده در بیمارستان ولی عصر (عج) تبریز انتخاب و با استفاده از تست های استاندارد آزمایشگاهی تعیین هویت شد. مقاومت جدایه ها به آنتی بیوتیک های متداول در درمان عفونت های استافیلوکوکی به کمک روش انتشار از دیسک و مقاومت نسبت به متی سیلین با استفاده از دیسک سفوکسیتین اندازه گیری گردید. وجود ژن های mecA و pvl در جدایه ها با استفاده از پرایمرهای اختصاصی و روش واکنش زنجیره ای پلیمراز (Polymerase chain reaction یا PCR) مورد بررسی قرار گرفت.
یافته هااز جدایه های مورد بررسی، 98 جدایه به عنوان استافیلوکوکوس اوریوس شناخته شد. بیشترین مقاومت آنتی بیوتیکی نسبت به پنی سیلین با 2/96 درصد و کمترین مقاومت به کوتریماکسازول با 0/16 درصد مشاهده شد. در 63 درصد از جدایه ها، مقاومت چندگانه آنتی بیوتیکی گزارش گردید. 26 جدایه به روش فنوتیپی، مقاوم به متی سیلین تشخیص داده شد. ژن mecA در 13 جدایه شناسایی گردید که 11 جدایه در بررسی فنوتیپی، مقاوم به متی سیلین و 2 جدایه، حساس به متی سیلین تشخیص داده شدند. ژن pvl در هیچ کدام از جدایه ها شناسایی نگردید.
نتیجه گیریوجود مقاومت بالا و چندگانه جدایه های استافیلوکوکوس اوریوس نسبت به آنتی بیوتیک های رایج و وجود جدایه های مقاوم به متی سیلین در بخش های مختلف بیمارستان، باید به عنوان یک موضوع جدی، مورد توجه و چاره اندیشی قرار گیرد.

Due to the negative impact of night and long shift work on performance of physicians, this study was conducted to determine the relation between shift duration with cognitive function and quality of life among emergency medicine professionals.
MethodsIn this cross-sectional study, 25 faculty members, residents, and specialists of emergency medicine were included, and their cognitive function [with Wechsler Adult Intelligence Scale-Fourth Edition (WAIS-IV)] was evaluated at the end of a 12-hour morning shift, a 12-hour night shift, and a long shift, and compared between the three shifts. The quality of life of individuals was assessed using the quality-of-life questionnaire of the World Health Organization, and the relationship between cognitive function and their quality of life was assessed.
FindingsThe mean score of cognitive function was 73.8 ± 3.58, 69.24± 4.73, and 59.8 ± 4.01 at the end of morning, night, and 24-hour shifts, respectively, and the difference between the three shifts was significant. The mean score of quality of life of the participants was 10.29 ± 80.52. A significant inverse correlation (0.386) was observed between the quality of life score and cognitive function in the 24-hour shift.
ConclusionFindings showed that the cognitive function of emergency medicine professionals at the end of 24-hour work shift decreased significantly compared to morning and night shifts. In addition, at the end of 24-hour shifts, there was a significant relationship between the score of cognitive dysfunction and quality of life.

In this research project, our aim was to evaluate and compare the effectiveness of autologous blood transfusion (ABI) and dry needling techniques in patients with refractory lateral epicondylitis.
MethodsThis study was performed as a randomized clinical trial on 32 patients with refractory or recurrent lateral epicondylitis. After enrolling in the study, patients were divided into two treatment groups. In the first group, patients were treated with dry needling method and in the second group, with ABI. Patients' pain was measured and compared by two methods of visual analog scale (VAS) and Patient-Rated Tennis Elbow Evaluation (PRTEE) before the interventions, and 1 and 3 months after the intervention.
FindingsBased on both criteria, 1 and 3 months after treatment, there was significant reduction in pain in both blood transfusion and dry needling groups. There was no significant difference between the groups in reducing pain over time based on VAS and PRTEE criteria.
ConclusionPain scores according to both VAS and PRTEE criteria significantly decreased in both groups at 1 and 3 months after the interventions.

Methicillin-resistant Staphylococcus aureus (MRSA) strains are known as a health problem in the world due to their resistance to most antibiotics. Pantone-valentine leucocidin (PVL) is one of the important toxins of this bacterium. The aim of this study was to determine the frequency of methicillin resistance, and to investigate mecA and pvl genes in Staphylococcus aureus isolates from Valiasr hospital in Tabriz, Iran.
MethodsIn this cross-sectional study during 2009-2010, 142 Staphylococcus isolates were collected from hospitalized patients and identified using standard laboratory tests. Isolate resistance to common antibiotics in the treatment of staphylococcal infections was evaluated by disk diffusion method, and resistance to methicillin was investigated by cefoxitin disk method. The presence of mecA and pvl genes in the isolates was evaluated using specific primers and polymerase chain reaction (PCR) method.
FindingsOf the studied isolates, 98 isolates were identified as Staphylococcus aureus. The highest antibiotic resistance level was observed against penicillin with 96.2%, and the lowest was determined against cotrimaxazole with 16.0%. Multidrug resistance (MDR) was observed in 63% of isolates. 26 isolates were identified as methicillin-resistance by phenotypic method. The mecA gene was detected in 13 isolates, of which 11 isolates were detected resistant and 2 isolates were sensitive to methicillin in the phenotypic tests. The pvl gene was not detected in any of the isolates.
ConclusionThe presence of high and multidrug-resistance to common antibiotics and the presence of methicillin-resistant isolates of Staphylococcus aureus in different wards of the hospital should be considered as a serious issue.
